Background: Treatment of musculoskeletal disorders by Traditional Bone Setters (TBS) is as old as man's effort at caring for his bone. In spite of the numerous limb-deforming and limb-threatening complications caused by this unorthodox method of care, the TBS still enjoy high patronage in the developing world. Aim: To evaluate the patronage and practice as well as the perception of patients receiving orthopaedic care at the University of Port Harcourt Teaching Hospital (UPTH) concerning traditional bone setters. Methods: A prospective non-randomized cross sectional study of 192 patients presenting to the University of Port Harcourt Teaching Hospital from Jan 2013 to July 2014 with complications following TBS care of musculo-skeletal problems was undertaken. Relevant data was obtained from each patient and analyzed. Descriptive statistics are presented in the results. Results: A total of 192 patients were seen; One hundred and fifteen (115; 64.6%) were males while seventy-eight (78; 35.4%) were female patients giving a male to female ratio of 1.5:1. Age of the patients ranged from 10months to 76years with mean age (35.1 ± 19.3) years. Patients aged Diamond et al.; BJMMR, 21(12): 1-8, 2017; Article no.BJMMR.34332 2 (21-30) years and (31-40) years age groups cumulatively constituted 42.6% of the study population. Students (65; 33.8%) and traders (41; 21.3%) were more common than other professions, with 73.4% of the population having at least secondary level of education. Following injury, more patients (78.6%, n=151/192) visited the TBS first than hospitals (10.2%, n= 41/192) with 46.4% (n=89) of patients visiting at least 2 TBS before hospital visit. The most common reason (n=82/192) for TBS visit was the strong faith patients had in the TBS. Conclusion: Strong faith in the competence of the TBS and perceived lower cost of care are key factors influencing perception and patronage of the TBS in Port Harcourt. Intervention programs need to address these key factors. Original Research Article

ABSTRACT… Objectives: To evaluate the factors for preferring the traditional bone setters by people. Study Design: Observational study. Setting: Department of Orthopedics and Spine Surgery Hayatabad Medical Complex Peshawar. Period: October, 2013 to March, 2014. Materials and Methods: Sixty consecutive patients who presented at orthopedics out-patient clinic after attending Traditional Bone Setting Centers were recruited for the study. Information about the patients' bio data, mechanism of injury, who advised them to contact TBS and what are the factors that compelled them to attend TBS were obtained and filled into prepared proforma. The data obtained was recorded and analyzed on SPSS version 17.Results: The mean age was 25.57 years with minimum age 5 years and maximum 65 years. Sixteen patients were females and 44 were male. Two (3.3%) patients first attended Modern orthopedic services (MOS) and 58(96.7%) patients attended TBS.47(78.3%) patients were taken by their own immediate family members to TBS, 20% were referred by friends and 1.7% were self-referral. Thirty seven (61.7%) opted for TBS because of their belief, 11(18.3%) considered this service cheap, 7(11.7%) attended it because it is quick service and 5 (8.3%) reverted to TBS because of attitude of hospital personals. Conclusion: A considerable of people still have great trust on TBS regarding management of musculoskeletal problems. Belief is the most leading cause of consulting traditional bonesetters, other causes include low cast, quick service and attitude of hospital personnel. External jig-aided intramedullary interlocking nailing of diaphyseal fractures: experience from a tropical developing centre

Intramedullary interlocking nailing for diaphyseal fractures is a standard treatment option in affluent societies. These procedures are often performed under image intensifier guidance. The cost of these gadgets precludes their common use in resource poor regions. External jig-aided intramedullary interlocking nailing is relatively cheap and offers the chance for performing these procedures in resource poor regions. The aim of this study was to document the advantages, challenges and outcome of this form of treatment in a resource poor setting. The Surgical Implant Generation Network (SIGN) implants and instrumentation were used for this study. Thirty-seven limbs in 35 patients were included. There were 30 males and five females giving a ratio of 6:1. The mean age was 35 ± 11.9 years with a range of 15-61 years. The femur and tibia were the bones studied with a total of 23 and 14 fractures, respectively (ratio 1.6:1). There were 14 comminuted fractures, two segmented fractures, six mal-unions, eight non-unions and seven simple fractures. Road traffic accidents were the most common cause of injuries with motorcycle accidents accounting for 19 (57.4%) cases. The mean follow-up period was 22 ± 5.32 months, mean time to union was 16.9 ± 5.4 weeks and the major complication was osteomyelitis (10.8%). We conclude that this is a viable treatment option for musculoskeletal injuries in resource poor regions. Education to encourage early acceptance of surgical intervention and reduced patronage of traditional bone setting for injudicious interventions can reduce the infective complication rates.
