Patient Safety Climate and Its Affecting Factors Among Rehabilitation Health Care Staff of Hospitals and Rehabilitation Centers in Iran-Tehran

Background: To achieve a safe and healthy environment for human beings and the dynamic industry as a critical factor in sustainable development, it is necessary to deal with accidents and eliminate their underlying causes radically. Safety climate shows the attitude and general perception of an organization and its management towards safety. This study aimed to investigate the effect of safety training on the safety climate of employees working in Kermanshah Province (Iran) Oil Refining Company. Materials and Methods: The present study was quasi-experimental interventional research conducted in 2019 in Kermanshah Province, Iran Oil Refining Company, with a pre-test, post-test design. The study population included all employees working in different departments of Kermanshah Oil Refining Company. Sampling was done randomly with 240 people. The questionnaire used in the present study had the demographic information questionnaire. To assess the workers’ climate safety, the safety climate questionnaire of the British Safety Organization was used. The level of reliability was 0.84. Information coding was performed in SPSS v. 23, and the Mean±SD of demographic information and atmosphere scores were determined. To investigate the effect of interventions on the safety climate before and one month after educational and technical interventions, a pairwise comparison was used. Results: The comparison of the mean scores of the safety climate components before the intervention was equal to 2.61 and after the educational intervention reached 3.72. All components of the safety climate increased after the intervention. The highest mean increase was related to the component of “safety communications” with 1.74 units. The lowest increase was associated with the component of “employee intervention in safety” with 0.68 units. Before and after the intervention, “safety enhancement” had the highest score among the safety climate components. Before the intervention, the lowest mean score was related to the “safety violations” component, which after the intervention, the “licensing system” had the lowest mean score. Conclusion: The educational intervention improved the safety climate in Kermanshah Oil Refining Company. Therefore, safety training plays an essential role in increasing the oil and gas industry’s safety climate.

Researchers have already highlighted the safety and health issues in workplaces. The necessity of these issues and their close relations to the environment have more transparently called for the management control of three critical dimensions of Health, Safety, and the Environment (HSE). Furthermore, following the rapid development of technology and the complexity of the activities tailored to the needs of companies and industries, a set of guidelines called HSE was developed to minimize the damages to human, equipment, and environment. Therefore, today presenting the academic education of HSE management system as a university Major, especially in universities affiliated with the Ministry of Health and Medical Education, like the University of Social Welfare and Rehabilitation Sciences seems necessary. In particular, USWR, besides rendering health, treatment, and rehabilitation services is a provider of the third level of the health system. Therefore, considering the importance of the issue, approximately a quarter century history of USWR establishment, and the fundamental nature of the health and prevention areas, this university attempted to use the comprehensive view of HSE management, to complete its activities in the field of Welfare and Rehabilitation Sciences. This paper aimed to investigate the necessity of establishing the HSE management Major at the USWR. The investigation was conducted using library review research, field study, community wisdom, and expert knowledge. The obtained data and results suggested that the USWR and its subsidiary centers recognized establishing HSE Major as beneficial in expanding research and science fields in rehabilitation services. It may also help pay more attention to the elderly, disabled, and children, both in terms of occupational hazards, and the general safety and various levels of life. Further in-depth research in these areas can ultimately help improve the wellbeing of all society members.

INTRODUCTION: Some working and organizational conditions, such as psychological stress and shift work, are factors that threaten the health of staff working in health centers. These factors can cause fatigue in a long time. Fatigue reduces the ability to process information and decrease to respond to hazardous conditions and will affect the safety of the environment. Therefore, the purpose of this study was to determine the relationship between safety climate and occupational fatigue in nurses working in Zabol city. METHODS: This cross-sectional study was performed on 143 nurses working in educational hospitals of Zabol in 2019. The proportional sampling method was used, and the Swedish Occupational Fatigue Questionnaire and the Nurses Safety Questionnaire were used for data collection. Data were analyzed using independent t -test, analysis of variance, Mann–Whitney, Kruskal–Wallis tests, and SPSS-21 software. RESULTS: In the present study, 57.3% were women and 42.7% were men. The mean score of occupational fatigue was 85.09 ± 41.49, and the mean score of safety climate was 67.15 ± 12.73. There is a significant inverse relationship between occupational fatigue and safety climate. The comparison of safety climate and its subscales between occupational and demographic variables showed that the score of male supervisors’ attitude was 01.36 ± 2.41 while the score of female supervisors’ attitude was 8.88 ± 2.61, and this difference was significant. Furthermore, cumulative burnout, the attitude of supervisors, and the safety climate were significantly different between different educational levels. CONCLUSION: The results of this study showed that there is a relationship between occupational fatigue and safety climate. Issues related to safety and risk factors in hospitals due to the high risk of disease outbreak and mortality, in addition to being economical, are important in terms of the human aspect as well. In addition, the activity of nursing staff is more important compared to other groups because of providing health care and communicating with patients; thus, more education about the safety climate of the workplace environment in hospitals can reduce nurses’ fatigue.
