Patient‐maintained propofol sedation using reaction time monitoring: a volunteer safety study*

Abstract: SummaryPrevious volunteer studies of an effect-site controlled, patient-maintained sedation system using propofol have demonstrated a risk of over-sedation. We have incorporated a reaction-time monitor into the handset of the patientmaintained sedation system to add an individualised patient-feedback mechanism. This study assessed if such reactiontime feedback modification would reduce the risk of over-sedation in 20 healthy volunteers deliberately attempting to over-administer themselves propofol. All the vol… Show more

“…However, oxygen desaturation and cardiac side events can occur with the use of propofol sedation during esophagogastroduodenoscopy (EGD) 1 , 2 . For the sake of safety, a pulse oxygen saturation of 90% or less during gastroscopy is considered to indicate hypoxia and require airway intervention to improve oxygen supply and prevent respiratory depression 13 , 14 . During painless gastroscopy, continuous oxygen supply and pulse oximetry are recommended for reducing the incidence of oxygen desaturation, and for the monitoring and timely detection of hypoxia, respectively 15 .…”

“…Also, the test has previously been applied to assess the sedative effects of benzodiazepines and propofol. 27,28 Limitations First, we included only male volunteers in order to standardize the experimental setup. The external validity in regard to gender may therefore be compromised.…”

“…If both conditions are met to allow a sedation increase, the software uses the Schnider model to calculate the required infusion rate to maintain an increase of 0.2 μg/ml in target Ce level (e.g. 0.5 to 0.7 μg/ml, as extensively studied and deemed appropriate in previous research [ 30 ]). This data is then outputted to the B. Braun Perfusor ® fm infusion pump through the serial port, and the pump responds accordingly.…”
