Patient–Doctor Relationship in Dermatology: From Compliance to Concordance

Abstract: The concept of what the doctor–patient relationship should be has changed increasingly in recent years. Previously, an asymmetric relationship was assumed. Compliance and adherence are terms used currently. The concordance model goes further and examines the effectiveness of the mutual process between the doctor and the patient. In this model the interaction is two-sided and involves finding a decision as partners. The origins of this approach are to be found in psychoanalytic theory.

“…The agreement is an essential and explicit attribute of concordance (Snowden & Marland, 2013 ). It occurs when the healthcare professional’s treatment plan is similar or harmonious to the patient’s opinion and lifestyle (Randall & Neubeck, 2016 ; Taube, 2016 ). The treatment plan is generated from goal setting and planning between the healthcare professional and patient, emphasizing patient-centeredness (Atal et al, 2019 ).…”

“…The patient-doctor relationship could be a pivotal aspect of the long-term treatment of chronic skin diseases ( Linder & Forchetti, 2009 ; Uhlenhake et al, 2010 ; World Health Organization, 2016 ), whose highly visible symptoms increase the likelihood of stigmatisation ( Barankin & DeKoven, 2002 ; Taube, 2016 ) and psychological distress ( Richards et al, 2004 ) and decrease quality of life ( Kowalewska et al, 2020 ; Miniszewska et al, 2020 ). However, there is little research on the doctor-patient relationship in dermatology and how it relates to post-visit outcomes (cf.…”
