The inflammatory process is a coordinated response that protects host after infection or trauma, involving several molecular reactions. Once the inflammation is closely linked to the process of destruction of the temporomandibular joint, this study aims to examine, by immunohistochemistry, the expression of interleukin-6 (IL-6), an important inflammatory marker, in temporomandibular articular discs of patients with anterior disc displacement with (ADDwR) and without reduction (ADDwoR) and its association with osteoarthrosis (OA). Thirty-eight (n = 38) articular discs were divided into two cutoffs: 1) analysis 1: 4 control (acute pathology), 17 ADDwR, 17 ADDwoR; and 2) analysis 2: without OA (n = 21) and with OA (n = 17). The area of immunostaining was compared statistically between groups (p < 0.05). In the disc samples, no significant differences were observed between the groups ADDwR and ADDwoR, and with and without OA, in respect to the expression of IL-6 by immunohistochemical examination. Future studies should be conducted with a larger sample size, which could clarify the association of the inflammatory mediator IL-6 with temporomandibular joint dysfunction.

ХИРУРГИЧЕСКИЙ РАЗДЕЛВ настоящее время роль хирурга в лечении патологии височно-нижнечелюстного сустава (ВНЧС) неоднознач-на. В соответствии с рекомендациями авторитетного аме-риканского профессионального сообщества AAOMS (American Association of Oral and Maxillofacial Surgeons), хирургическое вмешательство на суставе показано только в случае боли и (или) дисфункции, не отвечающих на кон-сервативные методы лечения. В связи с повсеместным развитием таких малоинвазивных методик, как артроцен-тез и артроскопия ВНЧС, на долю открытой хирургии приходится не более 10% всех хирургических манипуля-ций на данном суставе. Это объясняется также и тем, что в большинстве случаев симптомы дисфункции могут быть All the patients showed stability in skeletal and occlusal elements at 6 months follow-up period. The second group patients underwent disc-repositioning surgery only. In all the cases pain was completely eliminated but there was no sufficient improvement in MIO at 6 months follow-up period. Open TMJ disc repositioning surgery can be used for prevention of relapse after orthognathic surgery in patients with TMJ internal derangements such as anterior disc displacement without reduction.

Complex occlusal rehabilitation of a mutilated natural dentition in a patient whose temperomandibular joint is compromised by a systemic condition like rheumatoid arthritis and its drug regimen of immunosuppressant medication, would apprehend many practitioners and dental academicians to treat because of unpredictable prognosis. Pre prosthetic mouth preparation in such cases is unavoidable and requires surgical intervention in the form of pulp extirpation, crown lengthening and extraction of natural teeth. A female patient aged 27 years reported with such challenging medical characteristics. Organized surgical intervention and immune enhancing diet procured maximum benefits during the drug stoppage time. Foundation restorations included multiple cast dowel cores without having any guidance from adjacent teeth regarding their inclination and height. A new method of determining cast dowel height and inclination has been described. Porcelain fused to metal crowns and fixed partial denture completed rehabilitation with a mutually protected occlusion. The patient was highly satisfied with her redeemed appearance.
