ZusammenfassungIschämien des Rückenmarks sind selten. Akute/subakute Gefäßverschlüsse (mit der Folge einer akuten [AIM]/subakuten ischämischen Myelomalazie) müssen von chronisch-venösen Abflussstörungen (mit konsekutiver kongestiver Myelopathie) unterschieden werden. Das Ursachenspektrum einer AIM ist vielfältig und reicht von lokalen Veränderungen der Aorta und ihrer Äste (dissezierendes Aneurysma, Arteriosklerose, Aortenchirurgie, Vaskulitis) bis hin zu Kreislaufstillstand und Embolien von Bandscheibenmaterial. Die zumeist akut auftretende Querschnittssymptomatik wird in der Regel von initialen Schmerzen begleitet. Die MRT-Untersuchungen (Magnetresonanztomographie) zeigen im Verlauf Lokalisation und Ausdehnung der Ischämie; vorzugsweise imponieren stiftförmige Läsionen der stark vaskularisierten Intumeszenzen zervikal und thorakolumbal. Charakteristische Zeichen wie die Kontrastaufnahme der Cauda equina und der Wirbelkörperinfarkt sind im Verlauf pathognomonisch für die akute spinale Ischämie. Die spinale Angiographie kann wichtige zusätzliche und relevante Informationen über die spinale Gefäßversorgung liefern. Die postulierte Annahme einer sog. Wasserscheide mit einer erhöhten Vulnerabilität der oberen Thorakalregion für Ischämien ist nicht länger haltbar. Es ergeben sich starke Hinweise auf die pathogenetische Rolle von Bandscheibenvorfällen, die über fibrokartilaginäre Embolien zu erklären ist. Therapeutische Ansätze leiten sich aus den besonderen anatomischen Bedingungen spinaler Ischämien und der zugrundeliegenden Ätiopathogenese ab, die bei Berück-sichtigung der Anamnese, Zusatzdiagnostik, MRT und auch der spinalangiographischen Befunde zunehmend eingrenzbar ist.

Acute ischemic myelomalacia of arterial (and venous) origin has to be differentiated from chronic venous circulatory disorders resulting in congestive myelopathy. Many causes of acute spinal cord infarction have been described. They include diseases of the aorta and aortic surgery, systemic hypotension, and cartilaginous disk embolism. Acute ischemic myelomalacia normally has an acute onset, and pain is a common initial complaint. Magnetic resonance imaging (MRI) reveals the damage to the spinal cord at least in follow-up examinations.
