Pathogenic role of platelets in rheumatoid arthritis and systemic autoimmune diseases

Abstract: Well-recognized for their role in vascular homoeostasis, platelets may play a major role in inflammation and immunomodulation. Substantial data are emerging on the pathogenic involvement of platelets in inflammatory arthritis and autoimmune diseases, indicating the existence of crosstalk between the coagulation and inflammation system. Upon activation, platelets release pro-inflammatory platelets microparticles, which interact with leucocytes leading to joint and systemic inflammation in rheumatoid arthritis. … Show more

“…In the meantime these results were contradictory to others. 11,14,31 To what extent this controversy would hinder the inclusion of PIs in disease activity score of RA remains to be seen. This controversy would be similar or lower than the discrepancy between the objective parameter ESR or CRP and the subjective ones, number of swollen and tender joints and VAS in DAS28.…”

“…Thrombocytosis was described in RA patients during active stages of the disease, then counts decrease upon remission. 11 PIs are platelet parameters that could be easily obtained by modern automated blood cell analyzers. They include mean platelet volume (MPV), platelet count, platelet distribution width (PDW), plateletcrit (PCT), and platelet large cell ratio (P-LCR).…”

“…[22] The MPV is significantly lower in both RA and Ankylosing Spondylitis (AS) patients with active disease as compared with controls. [23] Other studies have shown that MPV is correlated with clinical disease activity indices in both RA and AS patients. [17,4] Moreover, platelet indices were substantially decreased after therapy.…”

“…The bubble-platelet interaction is regularly spotlighted in DCS. However, beyond its well-known thrombotic role, it would also be interesting to consider the involvement of the platelets in the immune response ( Harifi and Sibilia, 2016 ).…”
