Pathogenesis of giant cell arteritis with focus on cellular populations

Abstract: Giant cell arteritis (GCA), the most common non-infectious vasculitis, mainly affects elderly individuals. The disease usually affects the aorta and its main supra-aortic branches causing both general symptoms of inflammation and specific ischemic symptoms because of the limited blood flow due to arterial structural changes in the inflamed arteries. The pathogenesis of the GCA is complex and includes a dysregulated immune response that affects both the innate and the adaptive immunity. During the last two deca… Show more

“…Previous studies have analyzed immunological cellular activation and cytokines production in LVV [ 38 , 39 , 40 ], and others have studied the cause of the preponderance fibrous adventitial reaction in TAK patients. IL-6 production [ 41 ], activation of monocyte/macrophage lineage [ 42 , 43 , 44 , 45 ], high presence of activated mast cells [ 46 ], activated neutrophils [ 47 ], and the presence of Th17 lymphocyte populations [ 48 , 49 ] all seem to contribute to fibroblast activation and collagen deposition in the adventitial layer.…”

“…Vitamin D has immunomodulatory effects, including downregulation of Th1 and Th17 cells and upregulation of Treg cells [ 41–43 ]. Th1 and Th17 cell linages are crucial for initiating and maintaining arterial inflammation, while the levels of Treg cells are reported to be low in GCA [ 44 ]. However, the timing of a potential sun environmental trigger in the aetiology of GCA or PMR is not known.…”
