Pathogenesis of cervical artery dissections

Abstract: CAD is associated with ultrastructural connective tissue abnormalities, mostly without other clinical manifestations of a connective tissue disease. A structural defect in the extracellular matrix of the arterial wall leading to a genetic predisposition is suggested. The dermal connective tissue abnormalities detected can serve as a phenotypic marker for further genetic studies in patients with CAD and large families to possibly identify the underlying basic molecular defect(s).

“…(3) A skin biopsy from each patient was studied by light and electron microscopy in order to detect connective tissue alterations according to published criteria. 4,10 Microarray data sets were analyzed with different CNV calling algorithms. The Birdsuite algorithm yielded significantly more CNVs calls than PennCNV, with a higher proportion of Birdsuite calls being false positives, as was also found by others.…”

“…2 For this study we selected nontraumatic CeAD-patients from the databases of the University hospitals of Basel and Heidelberg who met the following inclusion criteria: (1) written informed consent, (2) Caucasian ethnicity (ie, both parents white Europeans), (3) information about the presence or absence of dermal connective tissue alterations based on criteria published previously 4,10 and (4) availability of sufficient DNA. Seventy-seven patients met the inclusion criteria.…”

“…The patient did not have signs of connective tissue disease but this diagnosis could not be completely ruled out. Ultrastructural connective tissue abnormalities have been described in skin biopsies of patients with cervical artery dissections who had no other clinical manifestation of a connective tissue disease 28 , alerting to phenotypical heterogeneity.…”

“…A apresentação mais freqüente é o acometimento unilateral da artéria carótida interna 4,5 . A dissecção multiarterial cervical ocorre em até um terço dos casos, eventualmente associada a algum defeito dos componentes do colágeno ou a uma arteriopatia que predisponha à ruptura da camada íntima ou média 6,7 .…”
