Pathogenesis, diagnosis and therapy of induratio penis plastica (IPP)

Abstract: Induratio penis plastica (or Peyronie's disease) is a rather rare malady, probably caused by chronic inflammation or recurrent microtrauma. The most common complaints are pain on erection and difficulties in performing sexual intercourse. Conservative treatment having been rather disappointing until now, infiltration with superoxide dismutase (Orgotein) seems to be an important advance. In more than one half of the patients positive effects concerning pain, deviation of the penis and extension of the plaques h… Show more

“…This may result from the inevitable accumulated sexual activity that increases the risk of repetitive penile trauma, or from the increased vulnerability of the tunica albuginea [22–24]. The results also confirm that β‐blocker therapy and diabetes seem to be potential risk factors [8,10,26], as the incidence of Peyronie's disease was three times greater in diabetics (18.3% vs 6.0%).…”

“…The most widely accepted hypothesis is that Peyronie's disease is initiated by trauma to the erect penis, with consecutive aberrant wound healing and scar formation [4–6]. Some authors suggested an infectious aetiology, autoimmune disease or local manifestation of a general fibromatosis [7–10].…”

“…Infection, autoimmune origin, local manifestation of a general fibromatosis and part of generalized arterial disease, have been suggested as probablee causes of PD. [710] A family history of PD in 2% of patients and an association with Dupuytren's palmar fibromatosis in 20%, which is a known inherited autosomal dominant disease, appear to suggest that patients may have an inherited predisposition to this disease. [8] In addition to a genetic element, an autoimmune component may be present, as evidenced by the finding of abnormal serologic tests in 785 men with PD[11] and the finding of elevated antielastin antibodies in the sera of men with the disease.…”
