Patent foramen ovale and neurosurgery in sitting position: a systematic review

Abstract: We have conducted a systematic review of air embolism complications of neurosurgery in the sitting position and patent foramen ovale (PFO) closure. It assesses the risk and benefit of PFO closure before neurosurgery in the sitting position. The databases Medline, Embase, and Cochrane Controlled Trial Register were systematically searched from inception to November 2007 for keywords in both topics separately. In total, 4806 patients were considered for neurosurgery in sitting position and 5416 patients underwen… Show more

“…The reported incidences of VAE in seated position surgeries vary depending on the type of surgery and the method of detection of VAE. In a systematic review of craniotomy studies, the incidence of VAE in seated position was 15% -45% (18). This almost high incidence of VAE observed in the present study could be related to the VAE detection technique, which was done by precordial Doppler that has a high sensitivity compared to other techniques, like ETCO2, SpO2, and ECG.…”

“…These include cryptogenic ischemic stroke, migraine headaches (especially those with aura), cerebral white matter changes detected by MRI, Alzheimer disease, sleep apnea, neurosurgery in a patient in a sitting position, and diving. [1][2][3][4][5][6][7] Although there is still controversy about the role of shunt closure for ischemic stroke in reducing recurrence, 8,9 it may reduce the frequency of migraine headaches. 10 An accurate diagnosis of RLS is important.…”
