Partial Epilepsy Manifesting Atonic Seizure: Report of Two Cases

Abstract: Summary:Purpose: Atonic seizures are commonly seen in patients with generalized epilepsy but only infrequently in patients with partial epilepsy. Clinically generalized atonic seizures as a partial epilepsy have not been studied in detail with video/EEG monitoring. Here we describe the clinical and physiologic characteristics of atonic seizures due to partial epilepsy and discuss the underlying mechanism. Methods: Two patients with partial epilepsy manifesting atonic seizures, one with frontal lobe epilepsy (FL… Show more

“…Hence, the slow falls in our patient agree with the atonic seizures that start partially, as reported by Satow et al, 15 who described partially starting atonic seizures in 2 patients with frontal and parietal lobe epilepsy. Fast activity with low voltage and rhythmic spikes was reported to have been observed in the patients' ictal EEG recordings.…”

“…Although atonic seizures are placed among generalized seizures, according to the International League Against Epilepsy, atonic seizures that start partially have also been reported. 14,15 Atonic seizures that start partially may be of 2 types. Although in the first one, sudden tonic posture forms before the fall, in the other, the fall occurs without any motor phenomenon.…”

“…A typical example is the successful surgery for patients with Lennox-Gastaut syndrome (LGS). It has been verified that a special group of LGS patients may have associated focal brain lesions (Bladin, 1985;Satow et al, 2002;Lee et al, 2014). Similarly, the aetiology of West syndrome has gradually changed from generalised seizures to seizures with multiple origins (Jonas et al, 2005;Komaki et al, 2001;Kang et al, 2006;Wyllie, 1996).…”

“…Inhibitory seizures are considered to be rare [35,36,37,38]. These ictal negative manifestations can be a neglect syndrome, apraxia, aphasia, homonomous hemianopia or a hemiparesis [39].…”
