“…Shortening the treatment time, reducing the normal tissue volume irradiated, and improving quality of life are major challenges already accomplished by multicatheter interstitial brachytherapy APBI [ 10 , 11 , 15 , 16 ]. This de-scalation in treated volumes should be strictly restricted to appropriately selected patients following the guideline recommendations of ESTRO [ 12 ], ASTRO [ 13 ], and ABS [ 14 ].…”

“…This is applicable not only for primary tumors, but also for cases of recurrence after a second conservative treatment [ 1 , 2 ]. Breast brachytherapy, encompassing accelerated partial breast irradiation (APBI) [ 3 ] or as a boost after whole breast radiation therapy (WBRT) [ 4 - 6 ], has been firmly established for selected patients, as recommended by international guidelines [ 7 - 10 ]. After a second conservative treatment, such as accelerated partial breast re-irradiation (APBrl) [ 11 ], it is also offered to patients with an ipsilateral breast tumor recurrence (IBTR) as salvage therapy.…”

“…OS, specific cause, local control, freedom from distant metastasis, and percentage of conversion to mastectomy are similar to total breast irradiation with 3D conformal RT 186 . Partial irradiation techniques include intraoperative RT, brachytherapy, and external conformal or intensity-modulated RT; each one with its respective dosimetric specifications and prescription regimens 187 . Outside of a clinical trial, candidates for partial breast irradiation include those aged ≥ 50 years, luminal subtype, ≤ 3 cm, absence of lymphovascular invasion, grade 1-2 invasive carcinoma, low-intermediate grade DCIS (≤ 2.5 cm with ≥ 3-mm margins), unicentric or unifocal lesion, negative margins (> 2 mm), negative lymph nodes (including isolated tumor cells), with no use of systemic therapy or neoadjuvant CT.…”
