Pars plana vitrectomy in the management of paediatric uveitis: the Massachusetts eye research and surgery Institution experience

Abstract: PPV is effective and safe in the management of chronic paediatric uveitis and its complications. It was able to reduce the amount of systemic medications required to control inflammation in this study. Patients with uveitis complicated by retinal vasculitis, however, are more likely to require long-term medical therapy to achieve inflammatory control.

“…Intraocular surgery may improve (Giuliari et al 2010;Quinones et al 2010) or exacerbate inflammation (Foster & Opremcak 2002) through activation of the underlying inflammatory process. Moreover, the procedure itself may result in an unusually severe inflammatory response, abnormal or excessive bleeding and ⁄ or unexpected postoperative intraocular pressure responses (hypertension or hypotony).…”

“…The secondary indications include two episodes of uveitis less than 2 months apart while under long-term immunosuppressive therapy and/ or intolerance of drug treatment. Vitrectomy treatments have been associated with especially good outcomes for childhood uveitis [6]. In the current report, the patient suffered from repeated attacks even under medical treatment for 1 year.…”

“…To avoid posterior capsular fibrosis and ciliary body detachment, cataract surgery in uveitic children usually includes a posterior capsulotomy and anterior vitrectomy as mentioned early [19,20,21,22]. An alternative approach, particular in eyes prone to severe inflammation, is to combine cataract extraction with a pars plana vitrectomy [20,21,23,24,25].…”

“…An alternative approach, particular in eyes prone to severe inflammation, is to combine cataract extraction with a pars plana vitrectomy [20,21,23,24,25]. A complete vitrectomy minimizes the risk of retinal complications such as peripheral tears in a way that is not possible using a purely anterior approach.…”
