Paroxysmal atrial fibrillation prevention by pacing in patients with pacemaker indication

Abstract: The prevent-AF registry demonstrated the usefulness of four preventive pacing algorithms in daily clinical practice. During the registry a significant reduction in AF burden and all other endpoints was observed. Dedicated diagnostics were key to adapting the optimal pacing therapy during follow-up.

“…Seventy‐nine consecutive patients (36 men, aged 77 ± 6 years) with symptomatic sinus bradycardia, New York Heart Association (NYHA) class I or II, no history of heart failure, and at least one documented AF episode during previous 3 months were enrolled between January 2004 and January 2005. They received a Selection 9000 (Vitatron) DDDR pacemaker equipped with diagnostic and preventive functions for AF 8–11 . Patients have not been previously diagnosed with OSA and have not received CPAP therapy.…”

“…Such a pacing function was developed to prevent AF episodes initiated by sudden onsets. However, no sustained clinical improvement has yet been shown for this algorithm 18–20 . Also in this study, the addition of continuous overdrive pacing to the triggered pacing functions had no influence on the frequency of sudden onsets and there was even a trend to an increased number of premature atrial complexes.…”

“…In two observational studies the activation of the triggered pacing function alone decreased the AF burden compared to the baseline measurement 19,20 . In contrast to these two studies, we have assessed this strategy with a randomized study design.…”

“…More recently, a variety of preventive atrial pacing strategies, including continuous overdrive pacing, pacing in response to atrial premature beats, postmode switch and postexercise pacing therapies, were developed to reduce the burden of AF among patients with known atrial tachyarrhythmias [68][69][70][71][72][73] . However, the magnitude of AF prevention due to dedicated preventive pacing algorithms and the identification of responder candidates remains unclear [74][75][76] .…”
