Objectives: The purpose of this study was to determine sonographically, in parotid glands of human immunodeficiency virus-positive patients, the condition of glands with or without enlargement, and propose a classification system for the patterns observed using diagnostic ultrasound imaging. Methods: In this prospective clinical study, ultrasound scans were performed on 200 patients aged 4-62 years at Mulago Hospital, Uganda. Results: There were four main distinct ultrasound pathological patterns in the parotids, i.e. lymphocytic aggregations (LAs), lymphoepithelial cysts (LECs), fatty infiltration (FI) and lymphadenopathy only. There were additional subdivisions depending on the presence of echogenic foci and intraparotid lymphadenopathy. Of those patients (n 5 64) without parotid enlargement, only 8% showed normal ultrasound features, whereas 34% showed LECs and 31% showed LAs. Of those (n 5 136) with parotid enlargement, 46% showed LECs, 23% showed FI and 15% showed LAs. The overall prevalence of LECs in the study sample was 42%. LECs were multiple, mainly between 7 mm and 12 mm in diameter and 26% showed internal echogenic foci either mobile or stationary. In contrast, LAs tended to be ill-defined, less than 5 mm and were not associated with posterior acoustic enhancement. Features differentiating LAs from LECs have not been previously described. Parotid FI (lipodystrophy) was noted in patients on highly active antiretroviral therapy, who showed lesser prevalence of LECs after 12 months of treatment. Conclusions: Our study of 200 patients is probably the largest such study in the English language literature. The wide spectrum of diagnostic ultrasound patterns was categorized into four main groups (ten subgroups).

Ces dernières années, la multithérapie associant différents antirétroviraux (ARV) est devenue la modalité de gestion de choix pour la prise en charge de l'infection à VIH. L'objectif de cette étude était d'évaluer l'effet des médicaments antirétroviraux sur la sécrétion salivaire chez des patients vivant avec le VIH. Matériel et méthodes : Une étude transversale comparative a été réalisée sur 166 patients vivant avec le VIH (dont 94 sous ARV et 72 ARV naïfs) à la clinique des Maladies infectieuses et tropicales, au CHUN de Fann (Dakar). Le débit salivaire non stimulé et le pH salivaire ont été évalués dans les deux groupes de patients. Les données ont été statistiquement analysées et exploitées en utilisant le logiciel SPSS. Résultats : Le profil sérologique des patients était largement dominé par le VIH-1 avec 87,1 %. Dans la population de patients vivant avec le VIH et sous ARV, 7,4 % présentaient une xérostomie et 5,3 % avaient une hyposialie et seule l'association thérapeutique TDF+3TC+NVP (Ténofovir + Lamivudine + Névirapine) était statistiquement associée à l'hyposialie. Par ailleurs, dans la population de patients vivant avec le VIH ARV naïfs, 20,8 % présentaient une xéro-stomie et 13,9 % une hyposialie ; le taux de lymphocytes T CD4 n'était pas corrélé à l'hyposialie. Conclusion : La fonction des glandes salivaires était beaucoup plus affectée chez les patients non traités ; est-ce à dire que le traitement ARV agit positivement sur la fonction sécrétoire des glandes salivaires ? D'autres études plus fines et sur des cohortes plus importantes devraient être envisagées pour confirmer cette tendance. On distingue des pathologies infectieuses, des néoplasies et l'hyperplasie lymphoïde kystique (HLK). Face à une sialomégalie persistante, l'hypothèse d'une HLK liée à l'infection par le VIH doit être évoquée. Le tissu lymphoïde des glandes salivaires représente une cible pour le VIH et son effet cytopathogène est directement à l'origine du développement de cette lésion. L'observation présentée est celle d'une patiente ayant une sialomégalie bilatérale. La démarche diagnostique a conduit à la découverte de sa séropostivité pour le VIH.

Parotid gland involvement in advanced AIDS

Abstract: These results indicate that infection and other lesions in the parotid glands are more frequent than hitherto described in the specialized literature in AIDS patients. Clinicians should consider parotid gland involvement, when evaluating disease extension in advanced AIDS patients.
