Ineffective handoff communication is a recognized patient safety risk in health care. In response, quality improvement experts have developed methods for structuring and standardizing transfer processes and handoff communication. These have included mnemonics for use during handovers, scripts or worksheets for the standardization of information conveyed, and the minimization of interruptions and distractions during verbal handovers. [1][2][3][4] Recent studies have also reported successfully leveraging multidisciplinary team strategies and workflow adjustments, such as reducing nighttime transfers from the ICU. 5 Most notably, the implementation of the I-PASS (illness severity, patient summary, action list, situation awareness and contingency plans, and synthesis by receiver)-supported handoff bundle studied with resident-to-resident handoffs has abstract BACKGROUND AND OBJECTIVES: Recent publications have shown improved outcomes associated with resident-to-resident handoff processes. However, the implementation of similar handoff processes for patients moving between units and teams with expansive responsibilities presents unique challenges. We sought to determine the impact of a multidisciplinary standardized handoff process on efficiency, safety culture, and satisfaction.

The concept of transition has growing significance within the healthcare settings. Healthcare professionals' encounters with patients and family during periods of health-illness transitions in addition to other transitions (e.g., developmental) can be significant. Therefore healthcare professionals should be aware of the meaning and the process of transition, to ensure efficient and competent provision of care. This paper will explore the concept of transition in the context of healthcare, and will suggest strategies to enable professionals to manage transition effectively.

The objective of study is to explore the experiences of parents taking care of their critically ill child at the Neonatal Intensive Care Unit (NICU) or the Paediatric Intensive Care Unit (PICU) in the Malaysian hospital. A total of ten parents were interviewed and selected for purposive sampling. The data was analysed using a thematic analysis based on the Colaizzi's approach. The study identified four emerging themes from data that included participation in care , participation in decision-making, challenges and coping mechanisms throughout the child 's hospitalisation. Parents have emphasised the importance of their participation in the care and decisionmaking of their child . They also have their own coping mechanisms that would make their journey less traumatic. Nurses need to enhance their communication skills and improve nurse-parent relationships.
