Parenteral versus early intrajejunal nutrition: Effect on pancreatitic natural course, entero-hormones release and its efficacy on dogs with acute pancreatitis

Abstract: AIM:To evaluate the effect of early intrajejunal nutrition (EIN) on the natural course, entero-hormone secretion and its efficacy on dogs with acute pancreatitis. METHODS:An acute pancreatitis model was induced by injecting 1 ml/kg of combined solution (2.5 % sodium taurocholate and 8 000-10 000 BAEE units trypsin/ml) into the pancreas via pancreatic duct. Fifteen dogs were divided into parenteral nutrition (PN) group and EIN group. Two groups were isonitrogenous and isocaloric. EIN was used at postoperative 2… Show more

“…Oxygen-derived free radicals and lipid peroxidation have been shown to play an important role in the pathogenesis of AP. 31 Similar findings were observed in this study. A severe increase was observed 24 hours later in serum amylase activity for the induced AP groups.…”

“…15,21 The levels of the pancreatic enzymes were found to be significant in acute cases, but decreased or normal levels can be possible in later stages. 31 This subject should be taken into consideration for the diagnosis and treatment of AP. Serum amylase level increased and then gradually decreased after 1 day.…”

“…As the average days of anorexia in all groups preceding hospitalization was 3 days, this finding suggests that the period of anorexia prior to hospitalization may be sufficient to "rest" the pancreas according to traditional dogma and that further imposed food restriction while hospitalized is unwarranted. Studies including dogs with both experimentally induced and naturally occurring AP report benefit from nutritional support implemented within 48 hours of admission or immediately if ≥5 days of anorexia, defined as consuming <66% of RER, is present . Early nutritional intervention becomes more important with increasing severity of disease .…”
