INTRODUCCIÓNEn 2007 la Sociedad Americana de Nutrición Parenteral y Enteral (ASPEN) recomendó la estandarización del proceso del manejo de la nutrición parenteral (NP), desde la prescripción a la administración, y el desarrollo de soluciones normalizadas. Si bien tenía en cuenta la complejidad de esto último en ciertos grupos de pacientes como los pediátricos y recalcaba que siempre debería existir la posibilidad de prescribir fórmulas individualizadas para determinados pacientes (1).
PRESCRIPCIÓNLa prescripción de NP es un proceso susceptible de errores. Un impreso de prescripción bien diseñado así como la informatización de la prescripción pueden disminuir la incidencia de errores y aumentar la eficiencia del procedimiento (2)(3)(4).El proceso de prescripción de la NP pediátrica no se ha investigado en todos sus aspectos ya que los estudios publicados centran la atención en definir los requerimientos nutricionales (3). Sin embargo, la prescripción es susceptible de incidencias como errores en el cálculo de los nutrientes, omisión de determinados aditivos o concentraciones inapropiadas de electrolitos (4). Al igual que ocurre en la prescripción de medicamentos, existe una mayor dificultad para detectar este tipo de problemas en los niños que en los adultos.Tradicionalmente la prescripción de NP se realiza completando un formulario preimpreso destinado a este fin, que se dirige al Servicio de Farmacia. Es el farmacéutico el que trascribe esta información a una aplicación informática que ayuda a la validación de la prescripción y gestiona la información necesaria para la elaboración. El impreso de prescripción tiene como objetivo la comunicación entre médico y farmacéutico, pero además, si está bien diseñado, evita errores y aumenta la eficiencia del proceso (4,5). En el impreso se debería considerar la inclusión de:-Fórmulas para el cálculo de los requerimientos energéticos y de líquidos según la edad y el sexo.-Composición de nutriciones estándar.-Requerimiento de los nutrientes según la edad para nutriciones no estandarizadas. -Protocolo de monitorización de pacientes. Hay distintas formas de realizar la prescripción de la nutrición pediátrica. La prescripción de los distintos componentes puede ser por kilo de peso y día, por cantidad total/día, por concentración (cantidad por 100 ml) o por volumen. Esta última puede ser útil cuando se trate de una fracción de una solución estándar (6).La prescripción de la NP ha de hacerse a diario tanto si es estándar como individualizada.En el momento actual estamos asistiendo a un gran desarrollo de las aplicaciones informáticas que permiten la prescripción electrónica de NP (7). Estas aplicaciones deben estar integradas con otras bases de datos de pacientes y de soluciones de nutrición, facilitando el proceso de prescripción por el médico y validación por el farmacéutico. La utilización de una aplicación para el cálculo, escritura y envío de la prescripción ahorraría tiempo y eliminaría tareas repetitivas y cálculos tediosos por parte del médico, así como la trascripción ...
