Parenteral Nutrition–Associated Hyperglycemia in Non–Critically Ill Inpatients Increases the Risk of In-Hospital Mortality (Multicenter Study)

Abstract: OBJECTIVEHyperglycemia may increase mortality in patients who receive total parenteral nutrition (TPN). However, this has not been well studied in noncritically ill patients (i.e., patients in the nonintensive care unit setting). The aim of this study was to determine whether mean blood glucose level during TPN infusion is associated with increased mortality in noncritically ill hospitalized patients.RESEARCH DESIGN AND METHODSThis prospective multicenter study involved 19 Spanish hospitals. Noncritically ill … Show more

“…A few differences between our results and earlier findings may relate to the fact that our population was clearly selected (only patients with an indication for TPN) and the contribution of other confounding factors like age, presence of cancer and other comorbidities and the presence of diabetes [30], all factors that can greatly contribute to malnutrition as well as mortality and that were controlled for in our study. On the other hand, the number of patients recruited, the use of national death registries to investigate mortality outcomes, and the fact that this was a multicenter prospective study all add value to the findings.…”

“…84 After allo-HSCT, PN-associated hyperglycemia is also common and was reported to be associated with infectious Act as ligands for PPAR-γ receptors enhancing insulin action in skeletal muscle and adipose tissue Little effect on insulin secretion Edema, heart failure, hepatotoxicity 101 Possible risk of bone fracture 102 Caution in long-term survivors with cardiac heart failure, hepatotoxicity or decreased bone mass Increased risk of bone fractures, particularly in patients who receive systemic corticosteroids, or those with reduced bone density α-Glucosidase inhibitors Effective to improve postprandial hyperglycemia…”
