ZusammenfassungKontroverse Literaturangaben und die Ergebnisse eigener Untersuchungen über die Auswirkungen einer unterschiedlichen Flüssigkeitszufuhr bei FlÜh-und Neugeborenen führten zur Entwicklung eines bedarfsangepaßten Infusionsschemas flir die Neonatalperiode. Die vorgeschlagenen Richtlinien belÜcksichtigen weitgehend den unterschiedlichen Wasser-und Energieumsatz bei Früh-und Neugeborenen. Die Elektrolytsubstitution wird grundsätzlich nicht auf das Körpergewicht sondern auf die jeweils zugeftihrte Infusionsmenge bezogen. Bedeutsam ist, daß kein Infusionsschema ohne ein entsprechendes Monitoring auskommt. Verlaufsbeobachtungen der wichtigsten Parameter im Serum und Urin an 65 ausschließlich parenteral ernährten Früh-und Neugeborenen haben gezeigt, daß nur sehr selten in besonderen Situationen geringe Korrekturen notwendig waren.On Parenteral Infusion Therapy Adapted to the Requirements of the Neonatal Period Controversial data in literature and the results of our own investigations of the effects of varying liquid supply in premature and newbom infants have caused the setting up of an infusion pattern adapted to requirements for the neonatal period. The guidelines as proposed largely take into account varying water and energy catabolism of premature and newbom infants. As a matter of principle electrolyte su bstitution is not related to body weight but to the particular infusion quantity administered. It is of vital importance that no infusion pattern can do without a corresponding monitoring. Observation of the most essential parameters in the sera and urines of 65 solely parenterally infused premature and newborn infants has shown that very rarely, only on special occasions, minor adjustments had to be made.Während über die Durchführung einer parenteralen Ernäh· rung bei Früh-und Neugeborenen jenseits der ersten Lebenswoche in der literatur eine relativ gute übereinstimmung besteht (2,3,7,10,14,18,19,23,39), gehen die Angaben über das ideale Flüssigkeits-, Elektrolyt-und Energieangebot in den ersten Lebenstagen erheblich auseinander (l, 4,9, 12,13,16,22,25,39, 40). Riegel betonte 1974, daß alle Dosierungsvorschläge fur Neu-und Frühgeborene nur empirisch definiert seien und exakte Untersuchungen zu diesem Problem seinerzeit noch nicht vorlagen. Aufgrund der von Hartmann (1968) gemachten Empfehlungen wurde in Lübeck ab 1970 einer relativ hohen Flüssigkeitszufuhr ab 1. Lebenstag der Vorzug gegeben, ohne daß aus der klinischen Erfahrung sichere Vorteile für dieses Vorgehen Klin. Pädiat. 193 (1981) abgeleitet werden konnten. Es wurden daher parallele Untersuchungen bei Früh-und Neugeborenen über die Auswirkung einer unterschiedlichen Flüssigkeitszufuhr in einer Dosierung von 60 oder 150 rn1/kg Körpergewicht und Tag bei gleicher Elektrolytsubstitution während der ersten 3 Lebenstage durchgefiihrt. über diese Untersuchung und ihre Ergebnisse ist bereits ausftihrlich berichtet worden (35).
