Parent verbal contingencies during the Lidcombe Program: Observations and statistical modeling of the treatment process

Abstract: Parents mostly provided the expected types of contingencies but the number was lower than expected. An unexpected association was found between number of verbal contingencies for stuttering and treatment duration. Further research is required to explore the relation between rates of parent verbal contingencies, treatment process duration, and treatment outcome.

“…Recent Lidcombe Program studies have begun to reveal challenges to the long-held theoretical belief that operant mechanisms drive therapeutic success. 86 89 Such discoveries are important, as knowledge about which components of a treatment are useful will enable ease of clinical problem solving and increase treatment efficiency. Furthermore, such knowledge may assist in individualizing treatment to better serve individual client differences associated with concomitant disorders and treatment-resistant cases.…”

“…According to the studies conducted in native English-speaking countries, the most frequently used parental verbal contingency is “praise,” and parental verbal contingencies given for unambiguous stuttering tend to increase the length of stage 1 [50]. Parental requests for self-correction, a parental verbal contingency given for unambiguous stuttering during Lidcombe Program implementation, were associated with longer treatment times [51].…”

“…Historically, there has been no effort to show that the child's stuttering rate can be varied if the contingency schedule varies, a major premise of most operant interventions. The more recent reports by Donaghy et al (2015) and Swift et al (2016) leave me even less sure how LP works, even when I see successful outcomes.…”
