Pararenal Leiomyosarcoma of the Inferior Vena Cava

Abstract: A complete surgical resection is the only proven therapeutic modality that prolongs the survival in patients with leiomyosarcoma of the inferior vena cava (IVC). Reconstruction of the IVC is not always necessary but is often required to facilitate venous drainage of the kidney for the tumors at the pararenal area of the IVC. Controversy exists in postoperative adjuvant therapy. Recently, we experienced four cases of pararenal leiomyosarcoma of the IVC, of which treatment consisted of a complete resection of th… Show more

“…Vascular leiomyosarcoma constitutes about 2% of all leiomyosarcomas and involves veins five times more than arteries (1,2). The diagnosis of vascular leiomyosarcoma is often missed because the clinical manifestations are non-specific and vary widely according to the location of the tumor, and because no reliable tumor marker is available.…”

“…Greater than 50% of all vascular LMSs occur in the IVC. 1 Prognosis is often poor due to advanced stage at diagnosis. Surgical excision of the tumor and affected IVC with negative margins is the only treatment shown to improve survival.…”

“…Radical surgical resection remains the only proven therapeutic modality that prolongs the survival in patients with leiomyosarcoma. 15,20 Histological grade often correlates with biological behaviour and with prognosis. The principal sites of metastasis are lungs and liver; regional lymph node metastasis are rare.…”
