Paraneoplastic subacute cutaneous lupus erythematosus associated with cholangiocarcinoma

Abstract: Subacute cutaneous lupus erythematosus (SCLE) is a dermatosis that occurs in genetically predisposed individuals. The exogenous stimulus that triggers this condition is usually unknown; however, medication is often implicated. Malignancy is a rare cause. We present a case of paraneoplastic SCLE to cholangiocarcinoma and briefly review the features of this interesting entity.

“…Several internal tumours have been associated with SCLE, including lung, breast, gastrointestinal, head and neck cancers, cholangiocarcinoma, and one case of in situ disease . Our cases were associated with aggressive or late‐stage tumours, with two of the three patients dying within 2 years, mirroring those reported . We propose that SCLE represents a late paraneoplastic sign.…”

“…Subacute cutaneous lupus and other connective tissue diseases have been reported as rare paraneoplastic syndromes in connection with lung, breast, gastric, hepatocellular tumors, non-Hodgkin lymphoma, laryngeal, uterine, esophageal adenocarcinoma and cholangiocarcinoma. 5,6 This is the first case, however, of LET associated with ALL. It is not known whether the hematological disease was present at the beginning of the course, or whether its diagnosis could have been delayed because of the corticosteroid treatment.…”

“…The present patient was finally diagnosed with ALL. Subacute cutaneous lupus and other connective tissue diseases have been reported as rare paraneoplastic syndromes in connection with lung, breast, gastric, hepatocellular tumors, non‐Hodgkin lymphoma, laryngeal, uterine, esophageal adenocarcinoma and cholangiocarcinoma . This is the first case, however, of LET associated with ALL.…”

“…Subacute cutaneous lupus erythematosus (SCLE) can rarely be a paraneoplastic phenomenon, appearing simultaneously with the onset of a cancer, resolving with successful cancer treatment and relapsing with recurrence of the malignancy, thus fulfilling McLean’s criteria for paraneoplastic dermatoses 1–6. While paraneoplastic SCLE is reported in association with many cancers, breast and lung cancer are the most commonly implicated malignancies 2–17. SCLE can be drug-induced, and proton pump inhibitors (PPIs) may be among the most common culprit medications 18 19…”
