Cholestasis is a general feature of intrahepatic or extrahepatic biliary obstruction by various mechanisms including cirrhosis, stricture, choledocholithiasis, hepatitis, and neoplasms. Neoplasms can directly impinge on the hepatobiliary tree resulting in bile stasis. Stauffer’s syndrome is another variant of this neoplastic process that can cause cholestasis and liver enzyme elevation without any direct hepatobiliary obstruction, and is thus categorized as a paraneoplastic syndrome of unclear pathophysiology. We report a first case of metastatic prostate adenocarcinoma with features of Stauffer’s syndrome that reversed completely on androgen deprivation therapy. This is in contrast to a previously reported case of Stauffer’s syndrome due to metastatic prostate adenocarcinoma, which reversed partially to androgen deprivation therapy. Our case demonstrates the importance of early recognition of Stauffer’s syndrome and underlying neoplasms in patients who present with cholestasis without clear evidence of intrahepatic or extrahepatic biliary obstruction, which may lead to early initiation of treatment.

Reversible intrahepatic cholestasis in metastatic prostate cancer: An uncommon paraneoplastic syndrome

Abstract.As with other genitourinary malignancies, a variety of paraneoplastic syndromes have been revealed to occur in patients with prostate cancer. Stauffer's Syndrome is a well-described clinical syndrome which manifests via intrahepatic cholestasis in patients with renal cell carcinoma. Less common is intrahepatic cholestasis occurring in association with prostate cancer. The current case report discusses a 67-year-old man presenting with liver failure secondary to intrahepatic cholestasis co-existing with metastatic prostate adenocarcinoma. The patient's liver failure completely resolved with androgen-deprivation therapy suggesting an association between these two entities. The case report evaluates existing literature on this uncommon syndrome including the clinical presentation, natural history, and potential pathophysiology.

A 68-year-old male was admitted to our hospital because of jaundice. Laboratory examinations revealed elevated total bilirubin (23.4 mg/dl) and hepatobiliary enzymes levels. Abdominal ultrasonography and computed tomography showed no abnormal finding in the liver and biliary ducts. Abdominal imaging examinations revealed a prostate tumor, and paraaortic and iliac lymph nodes enlargement. An elevated prostate-specific antigen (PSA) level of 15,018.0 ng/ml, followed by a positive prostate biopsy, confirmed prostate cancer. Histological examination of the liver biopsy showed non-specific inflammation of the portal area and sinusoid. We regarded the cholestatic jaundice as a paraneoplastic manifestation of the prostate cancer because of parallel dramatic decline of total bilirubin, hepatobiliary enzymes, and PSA levels after the initiation of anti-androgen treatment to the prostate cancer. Paraneoplastic syndrome should be included in the differential diagnosis of idiopathic cholestasis.
