Bladder paragangliomas are very rare tumors that are often misdiagnosed. The present study retrospectively analyzed the clinical data and follow-up results of patients with bladder paraganglioma that were treated surgically in Peking University Shenzhen Hospital between 2012 and 2018, and reviewed the relevant literature. A total of 4 patients with bladder paraganglioma were treated surgically from 2012 to 2018. All patients were female with ages ranging from 28 to 54 years old (average, 47.25). A total of one patient exhibited micturition attacks and palpitations, and one other exhibited gross hematuria. A total of two patients exhibited while no obvious symptoms. One patient exhibited elevated plasma catecholamine, was well prepared with medicine and underwent partial cystectomy, and 3 patients were not successfully diagnosed with paraganglioma before surgery. These patients received transurethral resection and 2 had distinct blood pressure elevation during operation. After surgery, the patients were followed-up for 5-69 months, during which all had normal blood pressure and none experienced recurrence. Bladder paragangliomas exhibit a variety of clinical presentations, which can be nonspecific for the non-functional ones, therefore may easily lead to misdiagnosis. Surgeons must fully consider the possibility of this disease when dealing with non-epithelial bladder tumors. Surgical resection is an effective treatment. For cases successfully diagnosed before surgery, partial cystectomy should be a treatment priority.

Introduction Paraganglioma of the bladder is an extremely rare tumor, and special attention should be paid to elevation of blood pressure during tumor resection. Case presentation A 64‐year‐old woman presented with elevation of blood pressure during thoracic surgery. Multiparametric magnetic resonance imaging revealed a bladder tumor, and noradrenalin levels were elevated in plasma and urine. The tumor was surgically removed by en bloc transurethral resection with laparoscopic partial cystectomy of bladder tumor. The margin of the tumor was easily identified, and perioperative blood pressure was stable. There was no evidence of residual tumor, local recurrence, or distant metastasis during 2‐year follow‐up. Conclusion To our knowledge, we report the first case of paraganglioma of the bladder resected by a minimally invasive and safe procedure: combination of en bloc transurethral resection and laparoscopic partial cystectomy.

Paraganglioma of the bladder is a rare entity, accounting for 0.06% of all bladder tumors. Case presentation: A 58-year-old woman had been annually followed up since being diagnosed with rectal cancer 5 years ago. In January 2018, follow-up computed tomography detected a bladder tumor, and she was referred to our department for a further examination. Cystoscopy revealed a submucosal tumor on her right bladder wall. We performed transurethral resection of the bladder tumor. When we first marked the tumor margin, the systolic blood pressure increased, so we abandoned resection. We performed meta-iodobenzylguanidine scintigraphy and acid urinary collection, neither of which revealed any abnormal findings. We therefore performed open partial cystectomy based on a clinical diagnosis of paraganglioma of the bladder. The pathological findings revealed paraganglioma of the bladder. Conclusion: We herein report a case of paraganglioma of the bladder. Keynote message1 Functional paraganglioma of the bladder is seen in both radiographic-negative and laboratory-negative cases.2 Partial cystectomy is a standard option for managing paraganglioma of the bladder.
