Papulonecrotic Tuberculid With Inflammatory Nodular Lesions of the Lower Leg

Abstract: Two cases of papulonecrotic tuberculid (PNT) with inflammatory nodules of the lower leg are reported. One is a 43‐year‐old woman with lymphadenitis tuberculosa, and the other is a 54‐year‐old man with penis tuberculid and epididymitis of possible tuberculous etiology. In both cases, PPD skin test was strongly positive, and antituberculous therapy was markedly effective. Although the theory of the etiology of PNT is changing due to the recent decrease in active tuberculosis, we think that one cause of so‐called… Show more

“…These findings support the coexistence of erythema induratum and papulonecrotic tuberculid. The simultaneous occurrence of both manifestations has been reported in the literature (2–10) (Table 1). Chuang et al (5) suggested that their coexistence may represent a morphologic continuum and different levels of immune complex–mediated vasculitis.…”

“…Papulonecrotic tuberculid is uncommon even in countries with a high prevalence of tuberculosis. It was reported in 10 of 222 patients with tuberculosis in South Vietnam (27); al the Osaka University School of Medicine in Japan it was diagnosed only 9 times during 15 years (7), and in 10 years only 37 cases were reported in all of Japan (19). Other series of patients were reported from South Africa (6), Nigeria (28), and Hong Kong (29).…”

“…A recently published monograph on histopathology of inflammatory skin diseases does not mention even the name PNT (2). However, proven cases of PNT are still being reported sporadically (3,4). We report a case of PNT because of its rarity and the controversy over its existence at the present time.…”
