Papillomas and carcinomas of the choroid plexus in children

Abstract: Total surgical excision is curative in cases of papillomas. For carcinomas, the most effective treatment remains total surgical excision; however, adjuvant treatment in the form of chemotherapy in patients younger than age 3 years, supplemented by radiation therapy in older children, can moderately reduce the risk of recurrence.

“…As a result, we recommend limiting the resection to avoid neurologic injury. Overall, the incidence of complications observed in our study compares favorably with other published studies [5,7,9,11], particularly those including patients treated in the pre-microsurgical era [6,12,13], in which perioperative mortality was high, especially in children [14][15][16][17][18][19]. Compared to published series from the modern microsurgical era, our observed morbidity and mortality were very minimal, once again confirming that these tumors can be safely resected in both children and adults [2,3,5,9,11].…”

“…Choroid plexus papillomas and carcinomas are rare neoplasms which account for less than 1% of all brain tumours; they are mainly located in the lateral and in the fourth ventricle [1,2,3]. …”

“…Nevertheless, the infant’s brain is able to compensate impairment of functional areas, often without persistent deficits [17, 18,] and subtotal extirpation, with the risk of malignant transformation, has a considerably poorer prognosis than total extirpation [1, 7, 8, 11, 14, 15]. Thus, in our opinion total extirpation should be the goal for treatment of infantile plexus papilloma, even if adjacent to functionally important areas.…”

“…It is widely agreed that in plexus papilloma, total extirpation should be the aim of surgery [1, 7, 8, 11, 14]. Moreover, total extirpation should be the goal in both ‘elective interventions’ and emergency surgery.…”
