Among 3205 patients who needed major gynecologic operations, only 250 (7.8%) had significant ovarian cysts or tumors (170 benign, 38 "low malignant potential," 42 gross cancers). Contrary to prior reports, this study show that most patients with ovarian neoplasia had had some symptoms (duration ranged from two weeks to ten years), or a palpable adnexal mass or both. Associated symptoms have been itemized in categories. Procrastination by patients, after occurrence of the first symptoms, resulted in the growth of later-stage cancers and lower survival rates. It may be inferred from age-incidence graphs that some benign cystadenomas may be present in ovaries for long intervals of time and then may become malignant. The importance of (a) careful recording of histories, (b) periodic pelvic examinations, (c) the use of preoperative methods of differential diagnosis and (d) the differentiation between gross tumor characteristics during operations are discussed. Earlier diagnosis and treatment of ovarian neoplasia are possible and will yield better long-term results.

The role of chemotherapy in the treatment of ovarian cancer is not independent of the other modalities utilized. Rather surgery, radiation therapy and chemotherapy must be integrated into a unified program. Some of the factors that we feel influence the role that each plays in the treatment of cancer of the ovary are discussed. Our program of therapy which integrates surgery, radiation therapy and chemotherapy, and matches their use to thestage of the patient’s disease is presented.

Electron microscopic evaluation was performed in a series of papillary serous tumors of the ovary, including 4 cystadenomas, 2 cystadenocarcinomas, and 2 tumors with questionable histologic evidence of malignancy. Marked nuclear irregularity was found in the cells of those tumors which were malignant by histologic criteria. Nuclei of cells from benign tumors had a regular shape and outline, with only shallow indentations. In the cases with borderline malignant changes, an intermediate degree of nuclear irregularity was present. Many of the cells showed a marked degree of nuclear infolding, comparable to that of the frankly malignant tumors. A decreased amount of cell membrane interdigitation was observed in the malignant tumors as compared to the benign. The borderline group was intermediate in this respect. Cilia were present in the benign and borderline malignant tumors but were not seen in the frankly malignant tumors. The findings indicate that electron microscopic analysis may be a useful adjunct in the evaluation of papillary cystic tumors of the ovary.
