Papillary mesothelioma of the tunica vaginalis propria testis

Abstract: Ultrastructural features of a papillary mesothelioma arising in a hydrocele-sack are reported. The tumour cells presented numerous microvilli, desmosomes, basement membranes and abundant bundles of microfilaments, which all are hallmarks of mesotheliomas. The predominant cell type was the "clear epithelial cell", but transitional cells and degenerative forms (foamy cells) were also found. The morphology and differential diagnosis of mesothelial tumours arising in the tunica vaginalis propria testis are discuss… Show more

“…11,13 To our knowledge, there have only been 11 well-documented cases reported as paratesticular 'well-differentiated papillary mesothelioma' to date. 3,6,8,12,[15][16][17][18][19][20][21] However, on the basis of microscopic descriptions and illustrations five had more complex areas within them, similar to the cases reported here. 6,12,16,18,20 Among the 11 cases, only one recurred and progressed to diffuse malignant mesothelioma.…”

“…Features include long, branching microvilli, abundant intracytoplasmic fibrils, desmosomes, and sparse intracytoplasmic organelles identical to those seen in mesotheliomas from the pleural cavities. 2,8,26,31,32 The differential diagnosis of paratesticular mesotheliomas is broad and includes both neoplastic and nonneoplastic conditions. Mesothelial hyperplasia, papillary serous neoplasms of the paratestis, tumors of the testicular collecting system (rete and epididymal carcinomas), and metastatic carcinomas may exhibit morphologic overlap with mesotheliomas of the tunica vaginalis.…”

“…However, the presence of lipid accumulation in carcinoma cells has not been demonstrated thus far. Several cases of mesothelioma with a foamy cytoplasm have been reported [12][13][14][15][16][17] , and intracytoplasmic lipid accumulation was reported in 2 cases [12,14] . In addition, in a case of papillary mesothelioma of the peritoneum [14] , cytoplasmic vacuoles containing lipid droplets were located in the basal portion of tumour cells that showed papillary growth.…”
