SUMMARYPancreas donor selection and recognition are important to cope with increasing organ shortage. We aim to show that the PDRI is more useful than the P-PASS to predict acceptance and should thus be preferred over P-PASS. Eurotransplant donors from 2004 until 2014 were included in this study. PDRI logistical factors were set to reference to purely reflect donor quality (PDRI donor ). PDRI and P-PASS association with allocation outcome was studied using area under the receiver operating characteristic curve (AUROC). Regional differences in donor quality were also investigated. Of the 10 444 pancreata that were reported, 6090 (58.3%) were accepted and 2947 (28.2%) were transplanted. We found that P-PASS was inferior to PDRI donor in its ability to predict organ reporting, acceptance, and transplantation: AUC 0.63, 0.67 and 0.73 for P-PASS vs. 0.78, 0.79 and 0.84 for PDRI donor , respectively. Furthermore, there were significant differences in donor quality among different Eurotransplant countries, both in reported donors and in transplanted organs. PDRI is a powerful predictor of allocation outcome and should be preferred over P-PASS. Proper donor selection and recognition, and possibly a more liberal approach toward inferior quality donors, may increase donation and transplant rates. Transplant International 2016; 29: 921-929

Amylase is measured with a rapid, easy and cheap laboratory test which is usually used in the differential diagnosis of the patients with abdominal pain. In addition to pancreatitis amilaz may increase in gastrointestinal, gynecologic, renal, cardiovascular and neurologic diseases so clinical situation of the patient should be assesed. In this article we intended to contribute clinicians knowledge about differential diagnosis of the patients with hyperamylasemia and pathogenesis of hyperamylasemia with current informations. The aim of this study was to establish how frequent the amylase test was ordered and to determine its effectiveness in patients that apply to the emergency department. Materials and Methods: Among the 32,922 patients who had amylase tests ordered, the data from the files of 4,318 patients with high amylase levels (>100 IU/L) were evaluated. The remaining 1,543 patients were included in the study. The demographic data, laboratory results, definitive diagnoses and medication data of the patients were evaluated. Results: Amylase tests were ordered in 15.1% of the patients that applied to the emergency department, and high amylase levels were detected in 1.98%. The incidence of acute pancreatitis diagnosed in the emergency department was identified as 60 in 100.000. When the serum amylase cutoff value was taken as three times greater than the normal upper limit, it was identified that the sensitivity and specificity of amylase in diagnosing acute pancreatitis was 85.4% and 98.0% when distinguishing patients with diseases known to increase amylase. Sensitivity and specificity of amylase in diagnosing acute pancreatitis was 85.4% and 95.3% when distinguishing patients with no diseases known to increase amylase or no medication use. Conclusion: Besides acute pancreatitis, increased amylase levels are also observed in other gastrointestinal, gynecological, cardiovascular, neurological diseases and renal dysfunction. Ordering amylase tests based on the patients' clinic will reduce ordering unnecessary tests in emergency departments.
