Painful testicular swelling: an unusual localization of sarcoidosis

Abstract: Sarcoidosis is a systemic inflammatory disease characterized by non-caseating epitheloid granulomas; whereas it usually involves the lungs and lymph nodes. Testicular sarcoidosis is extremely rare, having been reported to occur in 0.2% of all sarcoidosis patients. We describe a very unusual form of sarcoidosis of the testis, mimicking malignancy at initial presentation.

“…Perioperative biopsy with immediate fresh-frozen section evaluation can be performed even if the clinical picture is unclear and the risk of malignancy is intermediate. This will allow, if the malignancy is indeed confirmed, to proceed with the radical procedure immediately while minimizing the risk of spreading of potential malignant cells; on the other hand, this approach will prevent unnecessary orchiectomy if the finding is benign ( 2 , 3 , 6 , 7 , 9 , 10 , 13 , 14 , 16 , 19 , 20 , 28 , 30 , 32 , 35 ). Still, if the clinical picture strongly indicates malignancy, biopsy is unnecessary and a radical solution should be employed.…”
