Painful procedures and analgesia in the NICU: what has changed in the medical perception and practice in a ten-year period?

Abstract: Despite an increase in the medical perception of neonatal pain and in analgesic use during painful procedures, the gap between clinical practice and neonatologist perception of analgesia need did not change during the ten-year period.

“…Research indings emphasized that repeated exposure to painful stimuli during early stage of fundamental development of the nervous system leads to persistent behavioral and sensory changes [3]. Despite this fact, the use of appropriate pain relief interventions during potentially painful procedures is unusual in this population [4]. A paradox is still observed between the frequency of conditions that cause pain among young children and the use of appropriate pain relief intervention.…”

“…As a consequence, pain management is not completely effective. When investigating interventions in painful situations, the authors verified a divergence between the discourse and the practice of neonatologists, residents and medical students: although there has been an increase in awareness regarding the recognition and proper management of pain in neonates over the years, the performance of many procedures considered painful was not preceded by analgesia (3,14) . We emphasize the importance of the Brazilian version EDIN being incorporated by professionals who assist neonates, since accurate pain assessment is essential to guide treatment and pain management should be a priority in neonatal care in order to guarantee neonates a more humanized, safer and qualified care.…”
