Leading suicide theories and research in adults suggest that pain may exacerbate an individual's suicidal risk. Although pain and suicidality both increase in prevalence during adolescence, their relationship remains unclear. We aimed to systematically review the empirical evidence for an association between pain and suicidality in adolescence (PROSPERO: CRD42018097226). In total, 25 observational studies, published between 1961 and December 2018, exploring the potential pain-suicidality association in adolescence (10-19 years) were included. Across various samples and manifestations of pain and suicidality, we found that pain approximately doubles adolescents' suicidal risk, with a few studies suggesting that pain may predict suicidality longitudinally. Although depression was an important factor, it did not fully explain the painsuicidality association. Evidence on associations between pain characteristics and suicidality is sparse and inconclusive, potentially hiding developmental differences. Identification of psychological mediators and moderators is required to develop interventions tailored to the needs of adolescents in pain.

ObjectiveThis study examined the differences in the current levels of depression, anxiety, and physical pain in emerging adulthood among gay and bisexual men with various experiences of traditional and cyber homophobic bullying based on gender role nonconformity and sexual orientation and the moderating effects of family and peer support.MethodsA total of 500 gay or bisexual men (age 20–25 years) in Taiwan were recruited from August 2015 to July 2017. The levels of depression, anxiety, and physical pain among gay or bisexual men who had experienced both traditional and cyber homophobic bullying (n=109), only traditional or cyber bullying (n=173), and neither traditional nor cyber bullying during childhood (n=218) were compared. The moderating effects of family and peer support on the effects of homophobic bullying victimization on depression, anxiety, and physical pain were also examined.ResultsVictims of any type of homophobic bullying in childhood had more severe depression, anxiety, and physical pain in emerging adulthood than nonvictims. Victims of both traditional and cyber homophobic bullying had more severe anxiety in adulthood than victims of only traditional or cyber homophobic bullying. Family but not peer support in childhood moderated the effects of homophobic bullying victimization on current levels of anxiety and physical pain in emerging adulthood among gay and bisexual men.ConclusionThe results of the present study support that early prevention and intervention for homophobic bullying and enhancement of family support are essential to reduce mental health problems in emergent adults among gay and bisexual men.

Health-related quality of life in children, adolescents and young adults with self-harm or suicidality: A systematic review

Objective: Self-harm and suicidality are associated with substantial social and economic burden, especially among children, adolescents and young adults. The aim of this review was to systematically synthesize the literature on the association between health-related quality of life and self-harm/suicidality in children, adolescents and young adults. Methods: Searches were conducted via MEDLINE, PsycINFO, CINAHL, EconLit and EMBASE. Search terms were the combination of the following blocks: (1) self-harm/suicidality, (2) health-related quality of life/well-being/life satisfaction and (3) children/adolescents/young adults. The quality of studies was assessed using the Effective Public Health Practice Project tool. Results: We identified 23 relevant studies. Findings showed that participants who reported self-harm had lower well-being, life satisfaction or overall health-related quality of life compared to those without self-harm. There was also evidence supporting the association between health-related quality of life and suicidal attempt. However, the results for the association with suicidal ideation remained inconsistent. Additionally, mental health, emotional well-being, physical health, oral health, existential well-being and family quality of life were found to be significant domains associated with self-harm or suicidality. Regarding the quality of included studies, 35% ( n = 8), 39% ( n = 9) and 26% ( n = 6) of studies were scored as ‘Strong’, ‘Moderate’ and ‘Weak’, respectively. Conclusion: Findings from the review showed that health-related quality of life varied according to the severity of suicidality (from ideation to attempt). There was also no evidence to infer the direction of causality between health-related quality of life and self-harm/suicidality. The findings suggest a need for further research, in particular longitudinal studies to fill identified gaps in the literature.
