Pain management using patient controlled anaesthesia in adults post Nuss procedure: an analysis with respect to patient satisfaction

Abstract: Results obtained from retrospective patient satisfaction surveys indicate high efficacy for PCA as a pain management strategy. The large majority of patients did not seek more medication for pain alleviation and found in-hospital treatment sufficient. Further, all patients agreed that they were adequately informed of pain-risk prior to surgery.

“…There is also documented evidence pointing to the fact that there are numerous challenges with POP management that is more profound in developing countries as a result of inadequate resource allocations for pain management ( Kizza, Muliira, Kohi, & Nabirye, 2016 ; Murray & Retief, 2015; Woldehaimanot, Eshetie, & Kerie, 2014 ). Half the number of postoperative patients still report inadequate pain management despite efforts from worldwide authorities in creating awareness on the effects of inadequate POP management ( Admassu, Hailekiros, & Abdissa, 2016 ; Albrecht et al., 2013 ; Chou et al., 2016 ; Fletcher et al., 2015 ; Kolvekar, Pilegaard, Ashley, Simon, & Grant, 2016 ; Lorentzen, Hermansen, & Botti, 2012 ). Inadequate assessment and documentation is attributed to the high incidence of acute pain experienced by patients.…”

“…13 El consumo de opioides, a su vez, está asociado a efectos adversos como: náuseas, vómitos, retención urinaria y estreñimiento. 15,16 El manejo del dolor posterior a los procedimientos de Nuss, basado unicamente en analgésicos intravenosos, no ha arrojado beneficios claros. Los casos reportados suelen ser escasos y contradictorios, y no se considera una opción razonable, teniendo otros medios analgésicos al alcance.…”

“…However, this has fallen out of favor due to the incidence of rare neurologic complications (5,6) and the increased length of stay associated with epidural analgesia (3). More recently, many institutions favor the use of patient-controlled analgesia (PCA) with opioid medications (7). While opioids are effective, excessive use can have numerous adverse effects including nausea/ vomiting, constipation, urinary retention, and respiratory depression, as well as the potential for long term opioid dependence (8).…”
