Objectives: To study the prevalence of major depressive disorder, assess sleep quality, permanent functional disability as well as association of major depressive disorder with permanent disability, quality of life and quality of sleep in patients with chronic lower-back pain. Method: One hundred consecutive patients visiting an orthopaedic out patients department (OPD) with chronic lower-back pain in a tertiary hospital were included. They were interviewed for major depressive disorder (MDD) using DSM-5 and assessed for the severity of depression, health-related quality of life and sleep quality using the Hamilton Depression Rating Scale, Health Questionnaire Short Form (SF-36) and Pittsburgh Sleep Quality Index (PSQI), respectively. Proportion was compared using chi-square test, scores of scales were compared using the Mann-Whitney test and Kruskal-Wallis test followed by Dunn's post-hoc multiple comparisons. Statistical data were analyzed by Graph Pad InStat version 3.06.A p value of <0.05 was considered statistically significant. Results: The frequency of major depressive disorder and poor quality of sleep in patients with chronic lower-back pain was 74 % and 81% respectively. Patients with MDD had poorer quality of sleep, lower quality of life and higher functional disability. Severe functional disability due to lower-back pain was associated with severe depressive symptoms poorer quality of sleep and lower quality of life. The severity of depression is positively correlated with functional disability. Patients with a pain lasting more than 1 year had greater chances of depression. Conclusions: Patients with chronic lower-back pain had a high frequency of depression poor quality of sleep and lower quality of life, and higher functional disability. depression, lower-back pain, quality of life, chronic backache.

Development and Evaluation of a Screening Tool to Aid the Diagnosis of Cluster Headache

Cluster headache (CH), a severe primary headache, is often misdiagnosed and mismanaged. The aim of this study was to develop and evaluate a screening tool to aid the diagnosis of CH. We developed a novel 12-item screening tool. This was comprised of four components: (1) images depicting headache pain; (2) pain descriptors; (3) key questions that could differentiate between CH and migraine; and (4) a visual analogue pain scale. The total possible questionnaire score ranged from 3-32. Patients with CH and migraines (control group) were recruited prospectively from a headache centre in the North of England, UK. Two-hundred and ninety-six patients were included in the study: 81 CH patients, 36 of which suffer with episodic CH and 45 with chronic CH; 215 migraine patients, 92 of which suffer with episodic migraine and 123 with chronic migraine. The mean questionnaire score was higher in CH patients versus migraine patients (28.4 versus 19.5). At a cut-off score of >25 out of 32, the screening tool had a sensitivity of 86.4% and a specificity of 92.0% in differentiating between CH and migraine. The screening tool could be a useful instrument to aid the diagnosis of a CH. The images depicting headache pain do not clearly discriminate between CH and migraine.

Background A growing number of relevant studies on the comorbidity of pain and disability have been published in high-quality journals; however, few studies have analysed quantitatively the characteristics of the topic via bibliometrics. Purpose The study aims to systematically analyze various aspects of global scientific research on pain and disability comorbidity from 1980 to 2019 and determine the global research trend and knowledge structure. Methods A collection of publications on pain and disability comorbidity between 1980 and 2019 was retrieved from the Web of Science Core Collection. CiteSpace, Microsoft Excel 2016, and IBM SPSS Statistics 25 software were used to analyze publication outputs, authors, journals, institutions, countries, subject category, types of pain, references, and keywords. Results We collected a total of 3,570 articles in March 9, 2019. Results indicated that the publication outputs had a significant growth in the recent 40 years via the linear regression model (P<0.001). The journal Pain had the maximum output (178), the most cited frequencies in WoS (15,418), the highest impact factor index in 2017 (5.559), and the highest H-index (68). Linear regression analysis revealed that the tendency of multiple authors’ papers increased significantly (t=8.065, P<0.001). In accordance with the top 20 subject categories, Clinical Neurology was the most productive academic area (1095). With regard to types of pain, low back pain and headache were the most widely concerned with a total of 1,114 publications.Conclusion The study outlines the evolution and development of comorbid pain and disability research, reflects the overall situation and lays the groundwork for future research.
