Background:Patients with advanced cancer experience distressing symptoms and progressive decline in their performance status (PS) as death approaches. Aim: To identify the relationship between symptom burden and PS of Egyptian cancer patients receiving palliative care. Methods: This was a prospective observational study that included 100 patients with advanced cancer. Symptom burden was assessed using the Arabic version of the revised Edmonton Symptom Assessment System (ESAS-r). Performance status was assessed using the Palliative Performance Scale (PPS). Results:The mean total ESAS-r score was 60.1 (±10.7). The most common symptom to be reported as severe was pain (93%) followed by tiredness (74%), poor wellbeing (67%), lack of appetite (62%), anxiety (60%) and drowsiness (56%). The majority of patients (76%) had a PPS score ≤30 with an overall mean PPS score of 36.2 (±13.3). There was a statistically significant negative correlation between the total ESAS score and the PPS score (r = -0.687, p = < 0.001). Conclusion:The results suggest that Egyptian patients with advanced cancer experience high symptom burden and significant decline in PS. The higher the symptom burden, the poorer the PS of patients with advanced cancer.

Background/Aims To determine if there is a relationship between patient symptoms and functional improvement on inpatient rehabilitation. Methods Retrospective review of medical records at an American tertiary referral-based cancer center of all patients admitted to an inpatient rehabilitation unit between 3/1/2013-5/20/2013. Main outcome measures included the Edmonton Symptom and Assessment Scale (ESAS) and Functional Independence Measure (FIM). Findings The medical records for 71 unique cancer rehabilitation inpatients were analyzed. Statistical analysis of total admission ESAS on total FIM change found no significant relationships. The symptom burden of the patients was mild. Patients demonstrated statistically significant improvements in function and symptoms during inpatient rehabilitation. The mean change in total FIM and total ESAS were an increase of 19.20 and decrease of 7.41 respectively. Statistically significant changes occurred in fatigue, sleep, pain, and anxiety. Conclusion Both symptom and functional scores improved significantly during inpatient rehabilitation. However, no significant relationships were found between symptoms at admission and improvement in FIM.

Pain is a significant symptom in patients with chronic and life-threatening illness. While pain is traditionally thought of as a physiological experience, total pain recognizes the interplay of psychological, cognitive, social, spiritual, and cultural factors that influence the pain perception and total experience. Comprehensive pain assessment and management are foundational goals within the scope of palliative care, and optimal management depends on addressing each domain of the total pain experience. An overview of the total pain experience is provided, and clinicians should consider psychological, cognitive, social, spiritual, and cultural aspects in assessing pain. Pain management also addresses all domains, and suggestions are provided which address pain management barriers and challenges. First, patients should be educated about the benefits of pain management and importance to adhere to the plan of care. Second, healthcare professionals need education in order to manage pain properly and should adhere to internationally recognized evidence-based guidelines to provide care. Third, barriers to overcome system issues need to be addressed, such as working with governments and Ministries of Health to increase opioid availability for those in need and to ensure that patients can have access to opioids whether in the hospital, home, city, or rural area. While pain is a complex phenomenon, a comprehensive management plan can alleviate suffering for patients and their families.
