Pain, Depression and Quality of Life in Patients with Chronic Cervical Miyofascial Pain Syndrome

Abstract

ÖzetAmaç: Bu çalışmanın amacı kronik servikal miyofasiyal ağrı sendromlu (MAS) hastalarda depresyon, ağrı ve disabilite seviyesini araştırmak ve yaşam kalitesi ile ilişkisini belirlemektir. Gereç ve Yöntem: 40 kronik servikal MAS'lı hasta ve yaş ve cinsiyet açısından benzer 40 sağlıklı kontrol çalışmaya dahil edildi. Hastalar ve kontroller görsel anolog skala (GAS) ile gece, istirahat ve hareket ağrısı açısından değerlendirildi. Fonksiyonel disabiliteyi hesaplamak için boyun disabilite indeksi (BDİ) kullanıldı. Yaşam kalitesi kısa form-36 (SF-36) ile ölçüldü. Hastaların ve kontrol grubunun emosyonel durumları Beck depresyon ölçeği (BDÖ) ile değerlendirildi. Bulgular: Hastalar ve kontroller arasın-da demografik veriler açısından fark yoktu. Kronik servikal MAS'lı hastaların SF-36 skorları kontrollerden daha düşüktü. Hastalarda BDİ, BDÖ ve GAS skorları kontrollere göre yüksekti. Kronik servikal MAS'lı hastaların BDÖ skorları ile SF-36'nın alt parametreleri arasında negatif ve çok yakın bir ilişki bulundu (fiziksel işlevler (r:-0,599, p<0,001), fiziksel problemlere bağlı olarak rollerde engellenme (r:-0,558, p<0,001), bedensel acı (r:-0,540, p<0,001), genel sağ-lık algısı (r:-0,708, p<0,001), yaşam enerjisi (r:-0,692, p<0,001), sosyal işlev-ler (r:-0,559, p<0,001), duygusal sorunlara bağlı olarak rollerde engellenme (r: -0,537, p<0,001) ve ruhsal sağlık (r: -0,787, p<0,001). Tartışma: Kronik servikal MAS'lı hastalarda BDÖ skorları sağlıklı kontrollere göre daha yüksektir ve yaşam kalitelerini olumsuz etkiler. Kronik servikal MAS'lı hastaların psikiyatrik yönden değerlendirmeleri yaşam kalitelerini artıracak ve tedavi sonuçları-nın iyileşmesini sağlayacaktır. Material and Method: Forty patients with Cervical MPS and 40 age and sex-matched healthy controls enrolled in this study. The social and demographic characteristics of the patients and controls were examined. All patients and controls were evaluated with respect to pain (at night, rest and movement) and assessed by visual analog scale (VAS). Neck disability index (NDI) was used to calculate functional disability. Quality of life was evaluated with the the Short Form 36 Health Survey (SF-36). Also all of the patients and controls underwent Beck depression inventory (BDI). Results: There was no statistical difference between the patients and control cases according to demographical data. The SF-36 scores of the study patients were lower than controls. NDI, BDI and VAS scores were higher in the patients with chronic cervical MPS compared to controls.
