Pain is a common and distressing symptom in critically ill patients. Uncontrolled pain places patients at risk for numerous adverse psychological and physiological consequences, some of which may be life-threatening. A systematic assessment of pain is difficult in intensive care units because of the high percentage of patients who are noncommunicative and unable to self-report pain. Several tools have been developed to identify objective measures of pain, but the best tool has yet to be identified. A comprehensive search on the reliability and validity of observational pain scales indicated that although the Critical-Care Pain Observation Tool was superior to other tools in reliably detecting pain, pain assessment in individuals incapable of spontaneous neuromuscular movements or in patients with concurrent conditions, such as chronic pain or delirium, remains an enigma.

A valid pain assessment is the foundation of adequate pain management. Pain assessment can be challenging, especially in adult intensive care unit (ICU) patients who are unable to self-report. In such situations, relying on observational assessment tools is an alternative strategy. This review describes and analyzes the development and psychometric properties of pain assessment tools developed for use with nonverbal critically ill adults. A total of 32 relevant papers that described the psychometric properties of eight pain assessment tools were included. The scale development process, psychometric properties (i.e., reliability and validity), and feasibility of pain assessment tools were analyzed using a 0 to 20 scoring system. Each pain assessment tool was scored independently by two reviewers. Of the eight behavioral pain scales developed for use in adult ICU patients, the Behavioral Pain Scale (BPS) and the Critical-Care Pain Observation Tool (CPOT) are considered to be the most valid and reliable for this purpose, according to the available evidence. Behavioral pain scales may be viable alternatives to assessing pain in ICU patients who are unable to self-report, but only valid, reliable, and feasible scales should be used for this purpose.

ABSTRAKLatar belakang: Pasien kritis dewasa yang terpasang ventilator seringkali mengalami nyeri. Nyeri yang dirasakan pasien berbeda-beda dari tingkat sedang hingga berat. Faktor yang dapat mempengaruhi nyeri pada pasien kritis diantaranya jenis kelamin, usia, tingkat keparahan penyakit dan jenis penggunaan sedatif. Tujuan: untuk mengetahui demografi pasien kritis dewasa dengan ventilator yang mengalami nyeri. Metode: penelitian kuantitatif non-eksperimental dengan pendekatan crosssectional dengan 66 pasien kritis dewasa di ICU dari 3 Rumah Sakit di Semarang. Teknik pengambilan sampel dengan nonprobability sampling dan didapatkan 66 pasien kritis dewasa. Hasil: Hasil penelitian menunjukkan bahwa pasien kritis dewasa yang mengalami nyeri berusia 41-60 tahun sebanyak 38.3%, lebih dari 60 tahun sebanyak 38.8%, berjenis kelamin sebagian besar laki-laki sebanyak 66.7% dengan diagnosa medis sebagian besar adalah gangguan pernafasan sebanyak 45% dan tidak mendapatkan sedasi sebanyak 83.3%. Kesimpulan: nyeri yang dialami pada pasien kritis dewasa dapat diketahui dari demografi pasien diantaranya usia, jenis kelamin, diagnosa medis dan penggunaan sedasi. It is various among patient from moderate to severe. Factors that may affect pain in critical patients include gender, age, severity of the disease and type of sedative use. Objective: to determine the demographics of critically ill adult patients with ventilator who experience pain. Method: This is a nonexperimental quantitative study with 66 ventilated adult patient in the ICUs of the 3 hospitals in Semarang. Results: The results showed that critically aged patients with 41-60 years old pain experienced 38.3%, over 60 years of 38.8%, the almost gender is man of 66.7% with the most medical diagnoses were respiratory disorders by 45% and no use sedation is 83.3%. Conclusions: The pain experienced in adult critical patients can be seen from the demographics of patients such as age, gender, medical diagnosis and sedation use.
