Paget's disease of the esophagus

Abstract: We describe the case of a 72-year-old male patient who was suffering from Pagetoid spread to the esophageal mucosa from adenocarcinoma located in the distal esophagus and proximal stomach. Esophageal Paget's disease is rare. The histogenesis of extramammary Paget's disease has remained controversial. In the majority of cases, it originates from a primary esophageal tumor. In this report, we describe a patient who was suffering from esophageal adenocarcinoma and discuss the origin of extramammary Paget's diseas… Show more

“…According to several reports of primary esophageal Paget's disease, the patients' main complaints were dysphagia or chest discomfort similar to reflux esophagitis [2]. As Paget's cancer cells grow extensively superficially and multifocally into the mucosal and submucosal ducts without any protruded lesions, it is not so easy to diagnose this rare disease [3]. Endoscopic Lugol iodine spraying (Lugol chromoendoscopy) identifies Lugol-voiding lesions (LVLs), suggesting malignancy.…”

“…Conventional Lugol chromoendoscopy is used to diagnose the malignant potential lesion of the esophagus. This technique has some limitations for the correct assessment of Lugol-voiding lesions (LVLs) [3]. Magnified Narrow Band Imaging (M-NBI) endoscopy might be used for a better characterization of LVLs [4].…”

“…Background: Paget’s disease is thought to be a peculiar type of epidermotropic adenocarcinoma of mammary and extramammary tissues[ 80 ]. Extramammary Paget’s disease is most commonly seen in the vulva and anus, while an oesophageal location is exceedingly rare, with less than ten cases described[ 81 , 82 ]. Oesophageal Paget’s disease (OPD) always arises in the setting of Barrett’s oesophagus[ 81 , 82 ].…”

“…Clinical features and diagnostic approach: Typically, patients complain of progressive dysphagia, eventually accompanied by chest discomfort similar to reflux oesophagitis[ 81 , 82 ]. EGDS is a first-line diagnostic tool.…”

“…Having been first described in the breast by Sir James Paget in 1874, extramammary Paget’s disease is an entity most commonly seen in the vulva and anus but it is known to occur rarely in the esophagus, almost exclusively in the context of invasive esophageal adenocarcinoma. Paget’s cells within esophageal squamous epithelium are found in 4.9% of invasive adenocarcinoma cases and are most commonly associated with diffuse, poorly differentiated adenocarcinoma with dyscohesive malignant cells [25]. Few case reports are present in the literature to describe Paget’s cells within the esophagus but those that exist include cases involving mucous gland carcinoma, adenosquamous carcinoma and adenocarcinoma arising on a background of Barrett’s esophagus [25].…”
