Pacemaker Tachycardia in a Minute Ventilation Rate‐Adaptive Pacemaker Induced by Electrocardiographic Monitoring

Abstract: Programmed upper rate pacing occurred in a patient with a rate-adaptive pacemaker when he was connected to a cardiac monitor in an emergency department. The tachycardia was mistakenly interpreted to be ventricular tachycardia and the patient received multiple DC shocks as well as intravenous amiodarone and sotalol, resulting in severe hemodynamic deterioration. It is important that physicians working in the hospital environment be familiar with this pacemaker-monitor interaction as the problem may be easily re… Show more

“…An increase in the ventricular-atrial conduction time by some drugs including AV nodal blocking drugs 13 may also interfere with PMT termination algorithms. 13 The PVARP is the primary setting of a cardiac electronic device in preventing PMT. This preset allows the pacemaker to ignore any atrial impulses that occur within a specified time period after ventricular activation.…”

“…It should be noted that for many Guidant and/or CPI devices, Guidant Medical recommends increasing the pacing voltage to '5 volts or higher' when monopolar electrosurgery will be used. Sometimes, inappropriate therapy producing life-threatening results has been delivered in these settings [41,49 ]. Recently, pacing threshold was shown to be increased by disease states [39].…”
