Введение. Сочетание межпредсердной блокады (МПБ), обусловленной полной блокадой пучка Бахмана, с фибрилляцией предсердий (ФП), известное как синдром Байеса и активно изучаемое в последнее десятилетие, недостаточно изучено при сердечной недостаточности (СН). Цель. Изучение электрической предсердной дисфункции и распространенности МПБ у пациентов с СН, оценка атриальных факторов риска развития ФП у лиц с СН и у пациентов с имплантированными электронными кардиостимуляторами (ЭКС). Материалы и методы. В исследование включили 230 пациентов с синусовым ритмом и симптомами СН (ФК II–III по NYHA), в том числе 37 лиц с имплантированными двухкамерными (DDD) или ресинхронизирующими (CRT) устройствами ЭКС. Пациенты с ишемической болезнью сердца (ИБС: n=112) и дилатационной кардиомиопатией (ДКМП: n=118) были сопоставимы по полу (р=0,33), возрасту (р=0,41) и ФВ ЛЖ (р=0,23). Период наблюдения составил 2,4 [2; 3] года. При синусовом ритме анализировали исходные данные предсердной активации по результатам цифровой ЭКГ-12 (амплитуда, длительность, морфология Р-зубца и PR-интервал) и данным эхокардиографии. ФП-события оценивали с помощью серийного суточного мониторирования ЭКГ, ЭКС-интеррогирования (при наличии девайса) и по данным ЭКГ в динамике. Для многофакторного Кокс-регрессионного анализа в качестве первичной конечной точки были приняты ФП-события (впервые возникшая неклапанная пароксизмальная или персистирующая ФП). Результаты. При включении в исследование у 133 пациентов (57,8%) определена нормальная предсердная проводимость, у 36 лиц (15,7%) выявлена частичная МПБ, у 61 пациента (26,5%) обнаружена полная, или далеко зашедшая, МПБ. В результате корреляционного анализа выявлены значимые ассоциации между степенью МПБ и возрастом (р=0,001), продолжительностью СН (р=0,01) и коронарным генезом СН (р=0,001). У пациентов с ИБС (по сравнению с ДКМП) чаще наблюдалась аномальная межпредсердная проводимость в виде далеко зашедшей МПБ (34,8% vs 18,6%; χ2=7,78; р=0,006). В анализируемом периоде у 43 (18,7%) лиц впервые зарегистрированы эпизоды ФП (в том числе у 15 лиц с ЭКС). В результате анализа данных обнаружено, что ФП-события значимо чаще регистрировались у пациентов с расширенной Р-волной ≥130 мс (39,2% vs 3,62%; χ2=43,9; р<0,001), у пациентов с ИБС впервые выявленные ФП-события обнаружены значимо чаще, чем у лиц с ДКМП (25,9% vs 11,9%; χ2=7,55; р=0,006). В когорте пациентов с имплантированными ЭКС при анализе ЭКГ и эндограмм выявлены пролонгированные Р-волны с длительностью стимулированных зубцов Ppacing (Рр), значимо превышающей длительность синусовых Psinus (Рs). Более широкие синусовые Ps и стимулированные зубцы Pp наблюдались у пациентов с эпизодами ФП – ширина Рр составила 159±9,7 мс (против Ps 134±7,1 мс; р<0,001); у пациентов без ФП-событий длительность Рр составила 139±11 мс (против Ps 122±9 мс; р<0,01). В результате ROC-анализа показатель длительности стимулированной Р-волны (Рр ≥150 мс; AUC=0,902; р=0,0001) определен в качестве независимого предиктора ФП. В результате многофакторного регрессионного анализа Кокса обнаружены независимые ассоциации далеко зашедшей МПБ с эпизодами ФП (HR 5,14; 95% ДИ [2,68–10,8]; р<0,001). Выявлена экспоненциальная зависимость риска ФП от длительности Р-волны во II отведении ЭКГ. Заключение. У пациентов с СН частичная МПБ встречается в 15,7%, полная МПБ – в 26,5% случаев. Далеко зашедшая МПБ является независимым фактором риска ФП и ассоциирована с 5-кратным повышением риска ФП. Предсердная стимуляция значительно удлиняет пейсированный зубец Р, а расширенный стимулированный Pр ≥150 мс целесообразно рассматривать как новый критерий МПБ при наличии предсердной стимуляции; ширина стимулированной Р-волны ≥150 мс является независимым предиктором риска развития ФП у пациентов с имплантированными ЭКС.
Introduction. The combination of interatrial block (IAB) due to complete Bachmann bundle block with atrial fibrillation (AF) it’s known as "Bayes’ syndrome" that has been actively studied in the last decade but is still poorly understood for heart failure (HF). Purpose. To study electrical atrial dysfunction and the IAB prevalence in HF patients, to assess atrial risk factors for AF in patients with HF and in patients with implanted electronic pacemakers (IEPs). Materials and methods. The study collected 230 patients (pts) who had sinus rhythm and HF symptoms (NYHA II–III) including 37 pts with implanted dual-chamber (DDD) or resynchronization (CRT) pacing devices. Pts with coronary artery disease (CAD: n=112) and dilated cardiomyopathy (DCM: n=118) were matched by gender (p=0.33), age (p=0.41) and LVEF (p=0.23). The follow-up period was 2.4 [2; 3] years. In sinus rhythm, the initial data of atrial activation were analyzed according to digital ECG-12 (amplitude, duration, P-wave morphology and PR-interval) and echocardiography. AF events were assessed using serial 24-hour ECG monitoring, pacing interrogation (in case of the presence of a device) and according to dynamic ECG data. AF events (new-onset non- valvular paroxysmal or persistent AF) were taken as the primary end-pоint for multivariate Cox regression analysis. Results. At inclusion in the study 133 pts (57.8%) baseline had normal atrial conduction, 36 pts (15.7%) had partial IAB, and 61 pts (26.5%) had complete or advanced IAB. The correlation analysis revealed significant associations between IAB degree and age (p=0.001), long time of HF duration (p=0.01) and coronary cause of HF (p=0.001). Pts with CAD compared to DCM were more likely to have abnormal interatrial conduction as advanced IAB (34.8% vs 18.6%; χ2=7.78; p=0.006). In the analyzed period, 43 (18.7%) individuals had first-time AF events (including 15 pts with IEPs). As a result of data analysis, it was discovered that AF events were found significantly more often in pts with an extended P-wave ≥130 ms (39.2% vs 3.62%; χ2=43.9; p<0.001) and in pts with CAD new-onset AF events were detected significantly more often than in DCM pts (25.9% vs 11.9%; χ2=7.55; p=0.006). In the cohort of pts with IEPs, the analysis of ECG and endograms revealed prolonged P-waves with stimulated Ppacing (Pp) waves significantly exceeding the duration of native sinus Psinus (Ps). Wide both sinus Ps and pacing Pp waves were observed in pts with episodes of AF – the width of Pp was 159±9.7 ms (vs Ps 134±7.1 ms; p<0.001); in pts without AF events Pp duration was 139±11 ms (vs Ps 122±9 ms; p<0.01). As a result of ROC analysis, the duration of the stimulated P-wave (Pp ≥150 ms; AUC=0.902; p=0.0001) was determined as an independent predictor of AF. Multivariate Cox regression analysis found independent associations of advanced IAB with AF events (HR 5.14; 95% CI [2.68– 10.8]; p<0.001). An exponential dependence of AF risk was revealed depending on the duration of the P-wave measured in the II lead ECG. Conclusion. In pts with HF, partial IAB occurs in 15.7%, advanced IAB in 26.5% of cases. Advanced IAB is an independent risk factor for AF and is associated with a 5-fold increased risk of AF. Atrial pacing significantly lengthens the atrial P-wave and extended stimulated Pp ≥150 ms should be considered as a new criterion for IAB in the presence of atrial pacing; stimulated P-wave width ≥150 ms is an independent predictor of the AF risk in pts with IEPs.
