Two cases of oncocytoma of the parotid gland are presented, one of which was dominantly cystic and had recurred after 21 years. The light and electron, microscopic pictures are presented. The literature has been reviewed, and 131 cases were found in salivary glands; hence, oncocytomas in this location are rare. In older persons, they are found predominantly in the parotid gland, but have also been reported in the minor salivary glands. It is pointed out that electron microscopy or histochemical examination is indispensable for the demonstration of mitochondrial hyperplasia.

Background Oncocytomas are rare tumors; they occur most often in the parotid gland and here behave in a benign fashion. Few of these tumours have been described arising from tissues of the upper jaw. The reviews of the literature in these cases often appear incomplete. It has been suggested that outside the major salivary glands oncocytomas should be considered to be low‐grade adenocarcinomas. Methods We present a case of a 78‐year‐old woman with a frankly malignant oncocytoma of the lateral nasal wall, treated with radical local excision. A search of the world literature was undertaken and a review conducted. Results We found a total of 19 cases affecting the upper jaw, in a wide age distribution, 10 male and 9 female. Thirteen cases were malignant, 3 with metastases. Conclusions When affecting the upper jaw, these tumors must be considered malignant. Radical surgical excision is the treatment most often employed. Fifteen oncocytic (oxyphilic granular cell) neoplasms of the salivary glands collected from three large university medical centers are presented. The histologic and ultrastructural criteria that characterize oncocytes are presented and electron micrographs of two of our cases are discussed. Included in the series are 10 benign oncocytomas, two malignant oncocytomas and three benign, oncocytic pleomorphic adenomas. The clinical and pathologic features of benign oncocytomas are reviewed, with special attention to the variety of gross and microscopic changes present. The literature on malignant oncocytomas is critically reviewed, diagnostic criteria are evaluated, and a unique case is discussed in detail. Reference is made to a group of neoplasms that we designate as benign locally aggressive oncocytomas. The problem of differential diagnosis between oncocytoma and bening, oncocytic pleomorphic adenoma is considered.
