Lipoprotein (a) (Lp(a)) which is a low-density lipoprotein-like particle containing apo(a) is considered as an emergent cardiovascular risk factor. Type 2 diabetes mellitus (T2DM) is associated with a two- to threefold increase in the risk of cardiovascular disease (CVD). The aim of this study was to investigate the levels of Lp(a) in Black South African T2DM patients and its association with other metabolic factors. 67 T2DM patients and 48 healthy control participants were recruited for the cross-sectional study. The Lp(a) level was determined by ELISA and the result was analyzed using SPSS. The Lp(a) level in diabetics was found to be significantly increased (P = 0.001) when compared to the normal healthy group. In the diabetic group, the Lp(a) levels correlated significantly with the duration of diabetes (P = 0.008) and oxidized LDL (ox-LDL) levels (P = 0.03) and decreased total antioxidant capacity (P = 0.001). The third tertile of Lp(a) was significantly correlated with increased ox-LDL, C-reactive protein, and triglycerides and decreased total antioxidant capacity.

SUMMARY: This review highlights the human studies that explore the benefits of the Mediterranean diet and olive oil, based on gene expression analysis. We summarized consistent human transcriptomic studies on cardiovascular risk, based on TMD and olive oil interventions, with real life doses and conditions. A literature review was carried out leading up to February 2016. The results show that the TMD, specially supplemented with virgin olive oil, produces beneficial changes in the transcriptomic response of relevant genes in cardiovascular risk such as CAT, GPX1 and SIRT2. p65 and MCP-1, IL1B, IL6, CXCL1, INF-γ, ARHGAP15 and IL7R, which are involved in inflammation; and ABCA1, SR-B1, PPARBP, PPARα, PPARγ, PPARδ, CD-36 and COX-1, which play an important role in cholesterol efflux. The available data illustrate a transcriptomic effect on atherosclerosis, inflammation and oxidative stress pathways as well as the mentioned genes.KEYWORDS: Cardiovascular; Mediterranean Diet; MUFA; Nutrigenomics; Olive oil; Polyphenols; Transcriptomics RESUMEN: Protección cardiovascular de la dieta mediterránea y el aceite de oliva. Esta revisión resume los estudios de transcriptómica en humanos que muestran efectos beneficiosos de la dieta mediterránea tradicional (TMD) y el aceite de oliva, en condiciones y dosis de la vida real en relación al riesgo cardiovascular. La revisión se llevó a cabo hasta febrero de 2016. Los resultados muestran que la TMD, especialmente suplementada con aceite de oliva virgen, ejerce cambios beneficiosos en la respuesta transcriptómica de genes relevantes en el riesgo cardiovascular tales como CAT, GPX1 y SIRT2. p65 y MCP-1, IL1B, IL6, CXCL1, INF-γ, ARHGAP15 y IL7R implicados en la inflamación. ABCA1, SR-B1, PPARBP, PPARα, PPARγ, PPARδ, CD-36 y la COX-1 juegan un papel importante en el eflujo de colesterol. Además, ADRB2 está relacionada con el estrés oxidativo. Los datos disponibles nos llevan a un efecto transcriptómico sobre las vías de arteriosclerosis, inflamación y estrés oxidativo, así como sobre los genes mencionados.

The need to study cardiovascular diseases (CVD) has become more and more relevant as their prevalence has increased over the years. An intact endothelial wall is essential to vascular health. Certain factors are responsible for maintaining this tissue intact, including nitric oxide (NO), which provokes dilation of blood vessels in response to shear stress. Expression of the endothelial nitric oxide synthase (eNOS) enzyme, which produces nitric oxide in response to increases in blood flow, is of fundamental importance to maintenance of the vascular system. When this enzyme is inhibited, nitric oxide production is reduced, causing endothelial dysfunction. Since C-reactive protein inhibits production of nitric oxide by the eNOS enzyme, it is one of the causes of endothelial dysfunction and cardiovascular events. The objective of the present study was to review scientific articles in the literature related to the subject 'inflammatory markers and endothelial function' . A wide-ranging review of the current literature was conducted, using systematic analysis of bibliographic references indexed in PubMed, Scielo, Medline and LILACS database, for the years 1992 to 2013. The studies reviewed show that increases in inflammation causes reductions in NO and increases in cardiovascular events. Increased inflammation is associated with higher incidence of cardiovascular diseases.Keywords: inflammation; nitric oxide; endothelium. ResumoA necessidade de estudo das Doenças Cardiovasculares (DCV) vem à tona pelo aumento da sua prevalência ao longo dos anos. Uma parede endotelial íntegra é essencial para a saúde vascular. Alguns fatores são responsáveis pela integridade deste tecido, como o óxido nítrico (NO), que provoca a dilatação do vaso sanguíneo em resposta ao estresse de cisalhamento. A expressão da enzima óxido nítrico sintase endotelial (eNOS), que produz óxido nítrico em resposta ao incremento do fluxo sanguíneo, é fundamental para a manutenção do sistema vascular. Quando há inibição desta enzima, ocorre diminuição da produção de óxido nítrico, causando disfunção endotelial. A PCR inibe a produção de óxido nítrico pela enzima eNOS, sendo então uma causadora de disfunção endotelial e eventos cardiovasculares. O presente artigo tem como objetivo revisar artigos científicos na literatura relacionados ao tema 'marcadores inflamatórios e função endotelial' . Foi realizada uma ampla revisão de literatura atual, utilizando-se análise sistemática das referências bibliográficas nas bases de dados PubMed, Scielo, Medline e Lilacs, no período de 1992 a 2013. Os estudos revisados mostram que o aumento da inflamação causa uma diminuição de NO e aumento de eventos cardiovasculares. O aumento da inflamação está associado ao aumento da incidência de doenças cardiovasculares.Palavras-chave: inflamação; óxido nítrico; endotélio.
