Peritoneal dialysis (PD) is an alternative for managing the end-stage renal disease (ESRD). The peritoneal membrane (PM) is not just a membrane that passively responds to diffusion and convection. The characteristics of PM result in the peritoneal equilibrium test (PET) and with this test is possible to obtain the type of peritoneal transport (PT). The patient on PD can be classified in different types of PT as; Low, Low Average, High Average, and High. The aim of the study was to compare the inflammatory cytokines, oxidants, antioxidants, and oxidative DNA damage markers in the different types of PT. A cross-sectional analytical study of 77 adult PD patients was performed. Levels of lipoperoxides (LPO) were higher in all types of PT vs. healthy volunteer controls (HC) (p < 0.0001). Nitric oxide (NO) levels were found significantly down-regulated in all types of PT (p < 0.0001). The activity of the superoxide dismutase enzyme (SOD) was found to be significantly increased in all types of PT vs. the HC (p < 0.0001). The levels of the DNA repair enzyme were found to be decreased in all types of PT. The levels of the pro-inflammatory cytokines TNF-α, IL-6, the marker of oxidative DNA damage, 8-IP and the total antioxidant capacity (TAC) were all significantly decreased, contrary to the levels in HC, possibly by the clearance in the dialysis fluid in all types of PT or due to down-regulation of their expression. In conclusion, we found significant changes in markers of inflammation, oxidative stress, and oxidative damage to DNA in all types of PT; Low, low average, high average, and high PT in the values of D/P creatinine at 4 h compared to HC.

an excess of free radicals accompanies the development of renal pathologies and causes numerous concomitant complications and syndromes. the most common of these are cardiometabolic syndromes in patients with chronic kidney disease. therefore, the purpose of the study was to determine the activity of paraoxonase-1 and myeloperoxidase, which are associated with indicators of high-density lipoproteins content and oxidative stress in the blood of patients with the chronic stage of kidney disease. the activity of the enzymes, thiobarbiturate-active products concentration and transferrin, ceruloplasmin, thiol compounds content were determined in the blood of patients with chronic kidney disease. the oxidative status was shown to be changed. thus, myeloperoxidase activity, the content of oxidized proteins and the concentration of thiobarbiturate-positive components were increased, while the activity of the antioxidant enzyme paraoxonase-1, the content of transferrin, ceruloplasmin and thiol compounds were decreased. the ratio of myeloperoxidase / paraoxonase-1 activities was progressively increased up to 9-fold, indicating the presence of cardiovascular complications in patients. the data obtained allowed to extend the range of indicators for monitoring the development of cardiometabolic disorders in the progression of chronic kidney disease. k e y w o r d s: oxidative status, paraoxonase-1, myeloperoxidase, chronic kidney disease.

This study aimed to investigate the arylesterase activity of paraoxonase 1 (PON-1) in patients with end-stage renal disease (ESRD) and its relationship with oxidative stress markers. Methods. We conducted a one-time prospective observational study involving 58 ESRD patients. Among them, there were 20 hemodialyses (HD) patients and 38 patients treated with peritoneal dialysis (PD). The activity of PON-1 in serum was determined spectrophotometrically by the number of phenolic complexes formed using phenylacetate. Besides, spectrophotometrically the concentrations of malondialdehyde, serum concentrations of ceruloplasmin, thiol groups and total peroxidase activity (TPA) of erythrocyte were determined. The reference group consisted of 30 conditionally healthy individuals. Results. The arylesterase activity of the PON-1 in reference group was 6.57 kU/L versus 2.25 kU/L in HD patients and 4.26 kU/L in PD patients (p ˂ 0.0001). A direct correlation was found between arylesterase activity of PON-1 and ceruloplasmin concentration (p = 0.004), and TPA (p = 0.02) in HD patients. The activity of PON-1 in the serum of PD patients was associated with high-density lipoproteins (p ˂ 0.0001). Conclusions. We observed a decrease in the arylesterase activity of PON-1 in ESRD patients compared to the control group. The lowest activity of PON-1 is determined in HD patients. Moreover, the association of the PON-1 activity with a decrease in antioxidant blood markers was found. The enzyme activity in PD patients correlated with increased blood HDL. Further studies involving a larger dialysis cohort of patients are needed to determine the pathogenetic role of PON-1 activity in the development of cardiovascular events in ESRD patients.
