The painful peripheral neuropathy occurring frequently during chemotherapy with paclitaxel or oxaliplatin is one of their dose-limiting factors. We reported previously that substance P is involved in the pathogenesis of pulmonary hypersensitivity reaction to paclitaxel in rats, and an antiallergic agent pemirolast reverses this reaction via the blockade of release of substance P. In the present study, we investigated the involvement of substance P in paclitaxelinduced peripheral neuropathy compared with that by oxaliplatin. In von Frey and acetone tests in rats repeated administration of paclitaxel (6 mg/kg i.p., once a week for 4 weeks) or oxaliplatin (4 mg/kg i.p., twice a week for 4 weeks) induced both mechanical allodynia and cold hyperalgesia. Paclitaxel-induced peripheral On the other hand, oxaliplatin-induced peripheral neuropathy was not reversed by pemirolast. In the in vitro study using cultured adult rat dorsal root ganglion neurons paclitaxel (1000 ng/ml) significantly increased the release of substance P, and pemirolast (100 and 1000 nM) significantly inhibited this increase of substance P release. Oxaliplatin, by contrast, did not increase the release of substance P. These results suggest that substance P is involved in paclitaxelinduced neuropathy, and the mechanism of its action is clearly different from that of oxaliplatin.

За последние пару десятков лет мы стали свидетелями существенного прогресса в лечении метастатического колоректального рака: появление новых таргетных препаратов и дифференцированного подхода к их назначению, расширение показаний к резекции метастазов в печени и легких привели к увеличению продолжительности жизни пациентов этой группы. Тем не менее химиотерапия остается основой лекарственного лечения рака толстой кишки. В данной работе мы рассматриваем ряд новых подходов, касающихся применения химиотерапии при III и IV стадиях заболевания.Ключевые слова: метастатический колоректальный рак, рак толстой кишки, химиотерапия, оксалиплатин, 5-фторурацил, иринотекан. CHEMOTHERAPY IN THE MANAGEMENT OF LOCALIZED AND METASTATIC COLON CANCER: NEW AND YET UNFORGOTTEN APPROACHESOver the course of the past couple of decades, we have witnessed some significant improvements in the treatment of metastatic colorectal cancer: the emergence of novel targeted drugs and a differentiated approach to their administration, the extension of indications for surgical resection for liver and lungs metastases has resulted in the increased life expectancy among patients in this group. Nevertheless, chemotherapy remains the treatment of choice for colon cancer. In this paper, we discuss new approaches to the use of chemotherapy on stages III and IV of colorectal cancer.

Reintroduction of Oxaliplatin: A Viable Approach to the Long-Term Management of Metastatic Colorectal Cancer

Oxaliplatin-based chemotherapy is an effective first-line treatment option for patients with metastatic colorectal cancer (mCRC), which, in combination with targeted therapies and a sequential treatment approach using all active agents, has extended median overall survival to 2 years and beyond. Prolonged survival brings into focus the burden of treatment, in terms of associated toxicities and quality of life, and attention is now being paid to lowering the toxicity burden for patients receiving chemotherapy for mCRC without compromising efficacy. The use of oxaliplatin can lead to the development of sensory neuropathy, which commonly limits the dose and/or duration of treatment that can be administered. Temporary withdrawal of oxaliplatin (treatment holidays) has been shown to be an effective strategy for the management of this adverse effect. Data from randomized controlled trials indicate that a formalized stop-and-go approach to the delivery of oxaliplatin does not compromise efficacy, and indeed for some patients may prove beneficial by allowing them to continue treatment for longer periods. This paper presents a critical review of the evidence to support the utility of treatment interruption and reintroduction of oxaliplatin for the long-term management of mCRC.
