OXA-48 Carbapenemase-ProducingKlebsiella pneumoniaeIsolated from Libyan Patients

Abstract: Six multidrug-resistant Klebsiella pneumoniae isolates were recovered from injured Libyan combatants. Production of carbapenemase was screened by using commercial combination tablets from Rosco combined with a temocillin disk. Polymerase chain reaction (PCR) and sequencing were used to detect several carbapenemase genes and to characterize their genetic environment. Genetic support was studied by mating-out assays. Plasmid size was identified by the KADO method. PCR and sequencing allowed characterization of p… Show more

“…This sequence type was associated with various β-lactam resistance mechanisms in different parts of the world: CTX-M-15 production in France, Greece and Italy; SHV-12 production in the Czech Republic; OXA-48 production in Spain, Libya, France, Tunisia, Morocco, Switzerland and South Africa; KPC-2 production in Italy and Spain [32,[35][36][37][38][39][40][41][42]. In our institution the presence of CTX-M-15 producing ST101 seemed to be restricted both timely and spatially, as the majority of the isolates originated from a single department with a decreasing incidence ( Tables II and III).…”

“…The presence of 12 genes encoding the virulence factors MrkD, KfuBC, Cf29a, FimH, Uge, WabG, UreA, YcfM, EntB, YbtS, IroN, and AllS, were determined by PCR using previously described primers [23,24]. pLVPK-derived genetic loci (terW-iutA-rmpAsilS) and the repA gene were detected by PCR, using specific primers for terW, iutA, rmpA, silS and repA [25].…”

“…However, this hypothesis is not obvious considering that OXA-48-producing K. pneumoniae are very widespread in Turkey, and taking into account the heterogeneity observed in terms of antibiotic resistance for those isolates, the MICs of carbapenems, and coresistance markers (Table 2). Interestingly, ST101 and OXA-48-producing K. pneumoniae isolates have been reported in Libya (16) and were at the origin of an outbreak in Spain (17). In contrast, and even if OXA-48-producing K. pneumoniae isolates of many different STs have been identified in Turkey (18), that specific clone was never identified in that country.…”
