ContextCeliac disease (CD) is a common phenomenon in children with Type 1 diabetes (T1D). In the present review, we have discussed the pathogenesis, diagnostic biomarkers, risk factors, and prognosis of CD in the context of pediatric T1D.Evidence AcquisitionLiterature published in Web of Science, PubMed, Scopus, Google Scholar, and Cochrane Library were scrutinized up to the end of 2017. The keywords of celiac disease, Type 1 diabetes, children, and pediatric were used in different combinations.ResultsImmune cytotoxic reactions along with dampen immune regulatory functions contribute to CD in the context of pediatric T1D. Many children with simultaneous CD and T1D do not represent with the clinical signs of the enteropathy rendering a diagnostic challenge. The most common screening tests in these children are routine serological tests of CD, anti - endomysial, anti - transglutaminase, and anti - deamidated gliadin peptide antibodies. Typing for human leukocyte antigens of DQ - 2 and DQ - 8 may assist in the diagnosis of silent CD in children with T1D. The most significant shared non - HLA genetic loci of CD and T1D comprise CTLA - 4, TAGAP, IL - 18RAP, PTPN2, RGS1, SH2B3, CCR5. Interactions between these loci can be important in susceptibility to CD in T1D. Some new biomarkers have been suggested for diagnosis of CD including ischemia-modified albumin (IMA), soluble syndecan-1 (SSDC-1), regenerating gene Iα (REG-Iα), Neurotensin, and Zonulin, which can be useful for diagnosis and screening of CD in childhood T1D.ConclusionsOverall, active seropositive CD seems to be of clinical importance in T1D with significant impacts on the quality of life and predisposition to diabetes associated complications. It is important to detect CD in the context of T1D to prevent potential risks contributing to morbidities and mortalities associated with either CD or T1D.

Abstract:Background: Meniere's disease (MD) has been recently linked to gluten assumption. Approximately 75% of MD patients show positive skin test to food and about 50% of the positive responses are specific to the gliadin acid extract fraction. Aim of this study was to investigate the humoral immune responses to wheat antigens and related autoantigens in MD patients. Methods. We assessed the reactivity of sera from 28 patients with definite MD and 100 healthy controls against a repertoire of 51 antigens usually associated with immune reaction to gluten. Results. MD patients showed an increase of anti-wheat IgA, anti-cerebellar peptide IgA and anti-glutamic acid decarboxylase (GAD) 65 IgM compared to healthy controls. In particular, the increase of antiwheat IgA and GAD 65 IgM has been confirmed in a subgroup of MD patients symptomatically responding to a gluten free diet (GFD). Conclusion. In MD patients, an increase of the antibody production against gluten biomarkers was observed; in particular, anti-wheat IgA seems to be associated to clinical response to GFD.

Estes distúrbios são divididos em três entidades: doença celíaca (DC), alergia ao trigo (AT) e sensibilidade não-celíaca ao glúten ou ao trigo (SNCG/T), considerando que este cereal vem sendo associado mais frequentemente às manifestações, discutindo-se ainda se não seria o real componente alimentar desencadeante. A DC é uma enteropatia autoimune, enquanto a AT é uma reação de hipersensibilidade, mediada ou não mediada pela imunoglobulina E (IgE). A SNCG/T é uma entidade emergente, de fisiopatologia ainda discutida, que vários pesquisadores propõem como diagnóstico de exclusão em relação à AT e à DC. A SNCG/T vem despertando grande celeuma popular e polêmica nos meios médicos. A presente revisão tem por objetivo abordar a SNCG/T, procurando elencar elementos de distinção dos outros dois distúrbios retrocitados discutindo aspectos como características clínicas, diagnóstico diferencial, fisiopatologia e tratamento. Em muitos trabalhos, salienta-se a possibilidade de sobreposição ou de confusão diagnóstica com outras doenças funcionais gastrointestinais, como síndrome do intestino irritável (SII) e oligossacarídeos, dissacarídeos, monossacarídeos e polióis fermentáveis (FODMAPs, na sigla em inglês). Concluímos que a SNGG/T, embora constitua uma entidade clínica reconhecida por um número expressivo de pesquisadores, ainda é objeto de amplo questionamento quanto à sua caracterização, ao seu diagnóstico e à sua fisiopatologia. Isto posto, considerando sua elevada prevalência estimada e as repercussões populares, indubitavelmente trata-se de assunto digno de estudos mais aprofundados e bem conduzidos.
