Ovarian Tumors in Pregnancy

Abstract: The results of 90 pregnancies complicated by an ovarian tumor are analysed. On this basis it seems advisable to follow a 'wait-and-see' policy until the 16th week. Tumors persisting into the second trimester can best be extirpated between the 16th and 20th week. Ovarian tumors present after the first trimester can lead to serious complications. However, an ovarian tumor discovered in the second half of pregnancy calls for temporization of treatment. Cesarean section at term with simultaneous extirpation of the… Show more

“…The incidence of ovarian tumors during pregnancy is 1:80 to 1:5,000 pregnancies [1,2], probably equivalent to the incidence in nonpregnant women [3,4]. A metaanalysis of the larger studies shows an average incidence of 1:600 pregnancies [5].…”

“…Most corpus luteum cysts of the first trimester regress spontaneously. In case of persistence and corresponding criteria of malignancy in ultrasound, Struyk and Treffers [3] and Hess et al [10] recommend surgical removal during the second trimester to avoid any delay in the case of potential malignancy and to minimize the risk of complications such as torsion or rupture. The size of a tumor is doubtlessly associated with the likelihood of spontaneous remissions.…”

“…Trimenon, empfehlen aber die Operation bei Persistenz in der 16. und 19. SSW [5,6]. Dies wird damit begründet, dass Adnextumore in fast zwei Dritteln der Fälle während des weiteren Schwangerschaftsverlaufs zu Komplikationen Anlass gäben [6] (Tab.…”

“…Surgical removal of asymptomatic adnexal masses by laparotomy in pregnancy is traditionally postponed until 16-20 weeks of gestation, in order to decrease the abortion rate, which may increase due to the procedure [1,2]. During the last decade, two major changes concerning ultrasonography and ovarian cysts in pregnancy have occurred.…”

“…First, routine ultrasonographic examination during the first trimester of pregnancy has become very popular, resulting in the detection of many more asymptomatic ovarian cysts, which previously were not noticed. The reported incidence of ovarian tumors during pregnancy varies widely (1:81 to 1:2,500) due to different criteria and diagnostic techniques [2][3][4][5]. Most of these cysts are corpus luteum cysts which are related to pregnancy and usually resolve by 16 weeks of gestation [3,6].…”
