Ovarian ectopic gestation occurs when fetal tissue implants outside the uterine cavity. Implantation is usually tubal; Ovarian localization has an incidence of less than 1%. The diagnosis is confirmed by ultrasonography and the β subunit of chorionic gonadotropin in urine or blood; But its negativity doesn’t rule out the diagnosis. We present the case of a 19-year-old nulliparous patient with pelvic pain, with no significant history. Ultrasound revealed a 65 mm right ovary tumor. The β subunit value of chorionic gonadotropin was 3 mIU/L. The operative findings were: 100 cc hemoperitoneum, ruptured right ovary cyst of 6 x 4 cm ruptured, ectopic pregnancy of 7 weeks of gestation in Douglas sac fundus and phlegmonous appendix. The anatomopathological study reflected: moderate periappendicitis, right ovary with inflammation, neovascularization, vascular congestion, recent hemorrhage and ovarian ectopic pregnancy

ObjectiveTo compare clinical, ultrasound and biochemical characteristics of ovarian ectopic pregnancies (OEP) to tubal ectopic pregnancies (TEP).MethodsThis was a single‐center, retrospective, case‐control study of women with OEP compared to women with TEP between December 2010 and February 2021. OEP was defined as a pregnancy located completely or partially within the ovarian parenchyma, seen separately to a corpus luteum, where a corpus luteum was within the ipsilateral ovary. We compared demographic features, risk factors, clinical presentation, ultrasound findings and outcomes such as blood loss at surgery, blood transfusion rate, length of hospital stay, follow‐up and future pregnancy outcomes of women who conceived.Results20 women with OEP were identified and compared to 100 women with TEP. 15/20 (75%) OEPs were diagnosed correctly on the first scan. There was no difference between the groups in terms of maternal age, gestational age, gravidity, parity or risk factors. Compared to TEPs, OEPs were more likely to present with abdominal pain without vaginal bleeding (12/20 (60%) vs 13/100 (13%) (p=<0.01) (OR 10; 95%CI 3.45‐29.20)), were more likely to contain an embryo (3/20 (15%) vs 2/100 (2%) (p=0.02) (OR 8.7; 95%CI 1.34‐55.65)), have severe hemoperitoneum on ultrasound scan (9/20 (45%) vs 8/100 (8%) (p=<0.01) (OR 9.4; 95%CI 3.01‐29.40)) and had higher blood loss at surgery (median 700ml vs 100ml, p=<0.01). All surgically managed OEPs had successful laparoscopic treatment (18 excisions, 1 wedge resection) with preservation of the ovary. Only 1/20 (5%) OEPs required a blood transfusion.ConclusionsOEPs are more likely than TEPs to contain an embryo and to present with severe hemoperitoneum. In a dedicated early pregnancy setting the majority of OEPs could be detected on ultrasound scan at the initial visit, facilitating optimal minimally invasive surgical management, reducing the risk of blood transfusions and oophorectomy. Our findings can be used as a reference for clinicians who may not otherwise encounter this rare condition.This article is protected by copyright. All rights reserved.

ObjectivesA classification system of ovarian ectopic pregnancies (OEP) could aid in their management. This study aimed to investigate the ultrasound classification and preoperative ultrasound diagnosis of OEP.MethodsThis retrospective case series study analyzed the clinical and ultrasound imaging data of 68 pregnant women diagnosed and confirmed with OEP at the Obstetrics and Gynecology Hospital affiliated with Tongji University between January 2015 and March 2023. The study focused on examining the ultrasound characteristics of different types of ovarian pregnancies and the accuracy of preoperative ultrasound diagnoses.ResultsThe study included 68 patients with a median age of 30 (22‐47) years. There were 43 cases (63.2%) of ruptured OEP and 25 (36.8%) of unruptured OEP. The unruptured cases were subclassified as embryonic sac type (n = 16) and heterogeneous mass type (n = 9). Preoperative ultrasound accurately diagnosed 19 cases (27.9%) of OEP, including 10 (62.50%) of the embryonic sac type, 4 (44.44%) of the heterogeneous mass type, and 5 (11.63%) of the ruptured type. Preoperative ultrasonography showed that embryonic sac type OEP was characterized by an intra‐ovarian cystic structure with a distinctive hyperechoic ring and, in some cases (8 out of 16), the presence of embryonic buds and/or yolk sacs. Ruptured OEP was characterized by mixed masses in the pelvic cavity with minimal or significant free pelvic fluid, an unclear boundary between the mass and the ovary, and limited or no vascularity.ConclusionsUltrasound may be used to determine the type of OEP preoperatively, with a higher diagnostic yield for the embryonic sac subtype. Preoperative ultrasonographic classification may enhance the preoperative diagnosis of OEP.
